| Objective:(1)Study on the influencing factors of delayed graft function(DGF)in DD kidney transplantation.(2)study on the effects of delayed graft function on the postoperative hospital stay,half year renal survival rate,half year recipient survival rate and serum creatinine level.In order to take effective intervention measures in the process of donor maintenance,acquisition and patient management after kidney transplantation,reduce the incidence of delayed graft function,and promote the early recovery of renal transplantation function.Methods: In this study,70 recipients who received Deceased donor(DD)renal transplantation in our hospital from January 2017 to December 2018 were collected.According to the recovery of renal function,70 patients were divided into two groups:DGF group(n = 28)and IGF group(n = 42).The related factors of delayed recovery of renal function in DD donor kidney transplantation were analyzed by single factor analysis,and then the related factors of delayed recovery of renal function were selected according to the results of single factor analysis.The binary logistic regression model was used to analyze the multiple factors.The difference of serum creatinine level,receptor survival rate within half a year,and kidney loss rate within half a year between the two groups was analyzed The difference was statistically significant(P < 0.05).Results: among the 70 recipients included in this study,there were 28 cases of DGF,and the incidence of DGF was 40%.In the results of single factor analysis,the following variables showed significant statistical differences(P < 0.05): donor received CPR treatment,organ warm ischemia time ≥ 15 minutes,organ cold ischemia time ≥ 12 h,higher blood creatinine value of donor before entering the operate room,donor receiving CBP treatment.However,the age of donor,noradrenaline was used,the length of stay in ICU,the cause of death of donor were not the risk factors of DGF;except for the way of receptor dialysis,which was abdominal dialysis,which was the risk factor of DGF(P < 0.05),receptor gender,HLA mismatch number,dialysis Time and serum creatinine were not risk factors for DGF.The results of multivariate logistic regression analysis showed that the independent risk factors of DGF were CPR peritoneal dialysis,organ warm ischemia time ≥ 15 min,organ cold ischemia time ≥ 12 h.Compared with IGF group,the hospitalization time of DGF group was significantly longer.The levels of creatinine at the first month after surgery,creatinine levels at the third month after surgery,creatinine levels at the sixth month after surgery in DGF group were significantly higher than those in IGF group.The rate of kidney loss in DGF group was higher than that in IGF group within half a year.Compared with IGF group,DGF group had no significant difference in half year survival rate.Conclusion: The independent risk factors for DGF after DD donor kidney transplantation were: donor receiving CPR,recipient receiving peritoneal dialysis,organ hot ischemia time ≥ 15 min,and organ cold ischemia time ≥ 12 h.The occurrence of DGF prolonged the recipient’s hospital stay.The serum creatinine level of the DGF group was significantly higher than that of the IGF group,and the graft loss rate of the DGF group was higher than that of the IGF group. |
