Objective: The aim of this study was to investigate the changes of monocyte to high density lipoprotein ratio(MHR)in red blood cells,to investigate its correlation with the degree of coronary artery stenosis and other inflammatory indicators such as plasma fibrinogen,so as to provide theoretical basis for predicting the degree of coronary artery stenosis in patients with coronary heart disease.Methods: The study included a total of 326 inpatients who underwent coronary angiography for the first time in the First Affiliated Hospital of Guangxi Medical University from December 2018 to June 2019 due to chest tightness,chest pain and other symptoms.Among them,261 hospitalized patients with abnormal results indicated by coronary angiography were treated as the stenosis group,and the other 65 patients with normal coronary angiography were treated as the control group.According to the results ofcoronary angiography,the patients of stenosis group can be divided into high,medium and low Gensini score points.Collect the list general clinical data and laboratory test results and coronary angiography results,including coronary artery stenosis degree to evaluate the Gensini score system,using correlation analysis and regression model to evaluate MHR and Gensini score correlations between inflammation and other indicators,evaluation subjects work curve is used to its ability to predict high Gensini score.Results: Compared with the control group,the stenosis group had higher MHR value(p < 0.001),and the plasma fibrinogen level was significantly higher than that of the control group(p < 0.001),both of which presented an increasing trend with the improvement of Gensini integrand and the increase of coronary lesions.Correlation analysis results showed that MHR was positively correlated with Gensini integral(r =0.201,p <0.001),and plasma fibrinogen level was also positively correlated with Gensini integral.The binary Logistic regression model analysis results suggest after adjustment for age,sex,body mass index,smoking history,drinking history,history of hypertension,diabetes,dyslipidemia,fasting glucose,glycosylated hemoglobin,serum total cholesterol,serum triglyceride,low density lipoprotein cholesterol and other indicators,MHR and remain independent relationship between Gensini score(OR=9.535,95%CI 1.432-63.473,p=0.020).Using multi-factor linear regression analysis results suggest after adjustment for age,sex,body mass index,smoking history,drinking history,history of hypertension,diabetes,dyslipidemia,fasting glucose,glycosylated hemoglobin,serum total cholesterol,triglyceride,low density lipoprotein cholesterol after indicators MHR and plasma fibrinogen level(β= 0.267,p <0.001)between also shows an independent positive correlation.The results of the subject work curve analysis indicated that MHR could better predict high Gensini integral(AUC=0.621,95%CI 0.553-0.690,p =0.001),with an optimal threshold of 0.40,sensitivity of 89.5%,and specificity of 32.1%.When combined with fibrinogen,AUC increased to 0.728(p < 0.001).Conclusion: There was an independent positive correlation between MHR and the degree of coronary stenosis in patients with CORONARY heart disease,and MHR greater than 0.40 May indicate more severe coronary lesions in patients.There was also an independent positive correlation between MHR and plasma fibrinogen level,suggesting that there may be an internal relationship between MHR and fibrinogen. |
