Objective:To explore the correlation between blood samples collected from patients with acute severe cerebral infarction after admission,including inflammation indicators such as white blood cells and neutrophils,and the HS-mGPS scores evaluated by albumin and hs-CRP and the short-term prognosis of patients.Methods:A total of 111 patients with acute severe cerebral infarction who were hospitalized in the NICU ward of the Department of Neurology from January 1,2019 to April 30,2020 were collected and divided into 2 groups according to the 30-day outcome(survival or death),and 49 patients in the survival group.62 cases in the death group.All patients collected blood samples within 24 hours after admission,recorded platelets,leukocytes,neutrophils,lymphocytes,hs-CRP,albumin and other indicators,calculated NLR and PLR,and performed HS-mGPS score based on albumin and hs-CRP.Correlation analysis of various indicators between the two groups was carried out.Binary logistic regression analysis was used to explore the independent influencing factors of death.The ROC curve was used to analyze the predictive value of HS-mGPS,NLR,hs-CRP,etc.for the death of patients with acute severe cerebral infarction within 30 days.Result:1.There was no significant difference in age,ethnicity,hypertension,diabetes and whether it was recurrent cerebral infarction between survival group and death group(P>0.05).2.The gender,infarct volume and NIHSS score of the patients between the two groups were independent influencing factors(P<0.05).3.Compared between the two groups,the difference between white blood cell,neutrophil count,NLR,HS-mGPS score was statistically significant(P<0.05),the difference between albumin and platelet was not statistically significant(P>0.05).4.The results of the ROC curve for the prediction of the death of patients with acute severe cerebral infarction within 30 days by HS-mGPS score are as follows:HS-mGPS score AUC is 0.627(95%CI:0.524-0.731 P=0.022),cut-off value is 1,sensitivity is 25.81%,with a specificity of 91.84%.5.The ROC curve results of various inflammation indicators for the death of patients with acute severe cerebral infarction within 30 days are as follows:NLR(AUC 0.658),neutrophils(AUC 0.657),hs-CRP(AUC 0.637),white blood cells(AUC 0.634).Conclusion:1.HS-mGPS score,white blood cells,neutrophils,hs-CRP and NLR have predictive value for the death of patients with severe cerebral infarction within 30 days.NLR has advantages over neutrophils alone.2.Gender,infarct volume and NIHSS score are independent factors that affect the death of patients. |
