Clinical Analysis Of MIPPO Double Plate In The Treatment Of Schatzker Ⅴ And Ⅵ Tibial Plateau Fractures

Objective:To compare the effect of MIPPO(Minimally invasive percutaneous plate osteosynthesis)double plate technology and traditional open reduction treatment in the treatment of Schatzker V and VI tibial plateau fracture.Methods:27 patients with tibial plateau fracture(SchatzkerV and VI)admitted to the department of bone and joint Surgery of Affiliated Hospital of Yanbian University from January 2017 to June 2019 were analyzed,including 11 patients treated with MIPPO technology(experimental group)and 16 patients treated with traditional open reduction(control group).SPSS26.0 software was used for data statistics.The measurement data between the two groups were compared by Independent sample T-test,the statistical indicators were expressed as mean ±standard deviation,the disordered count data were tested by Fisher’s accurate test,and the ordered count data were tested by rank-sum test.P<0.05 was considered statistically significant for the difference between the two groups.Analysis and statistics of patients’ general information before treatment,fracture healing time,knee Rasmussen function score at 1,3,6,and 12 months after operation and VAS vision at 1,5,and 7 days after operation Pain score,statistically analyze the excellent rate of the two surgical methods and make an analysis and comparison.Results:All patients were followed up for 12-16 months,There was no significant statistical significance in gender,age,injury factors,affected side,fracture type and other general data of the two groups(P>0.05).The postoperative fracture healing time of the experimental group was significantly shorter than that of the control group(shortened from 30.25±1.69 weeks to 22.91±2.51 weeks),and the results after independent sample t test were statistically significant;the postoperative VAS score of the experimental group(postoperative 1,5 and 7 days were 4.64±0.81,3.18±0.60 and 2.09±0.70 points respectively)were significantly smaller than the control group(1,5,and 7 days after the operation were 6.75±0.93,4.69±0.79 and 3.31±0.70 points respectively),The results after independent sample t-test were statistically significant;the postoperative Rasmussen score of the experimental group(21.55±5.01,24.18±4.81,24.55±4.55 and 26.64±3.67 points at 1,3,6,and 12 months after surgery)Significantly higher than the control group(12.38±4.29,16.94±4.89,18.13±4.51 and 22.94±4.99 points at 1,3,6,and 12 months after operation),and the results after independent sample t test were statistically significant;The postoperative excellent and good rates of the experimental group(72.72%,81.81%,90.90%,and 90.90%,respectively,after 1,3,6,and 12 months)were also significantly higher than those of the control group(18.75%,37.50%,62.50%,and 81.25%),the results of the two groups were statistically significant after the rank sum test(P<0.05).Conclusion:The treatment effect of MIPPO double plate in the treatment of complex tibial plateau fracture is better than the traditional open reduction and internal fixation method,which can speed up the healing time of fracture,ensure the recovery of knee joint function,and improve the prognosis of patients.It is a feasible surgical method.
