Minimally Invasive Percutaneous Plate Osteosynthesis(MIPPO) Technology Combined With Locking Compress Plate (LCP) Internal Fixation Versus Expert Tibial Nail(ETN) In The Treatment Of Middle And Distal Tibial Fracture In Adults

ObjectiveTo investigate the operative time,the use of intraoperative fluoroscopy,the operative blood loss,time of fracture healing,the clinical,radiographic results,complications rates,between expert tibial nail and MIPPO technology in combination with LCP plate fixation for the treatment of the middle and distal tibial multiple fractures,and provide reference for clinical doctors.MethodThrough retrospective analysis of 89 patients with the middle and distal tibial multiple fractures of the Orthopaedic Department of the First Affiliated Hospital of Zhengzhou University from January 2014 to January 2017,we investigated the use of expert tibial nail and MIPPO technology in combination with LCP.The clinical efficacy of two surgical methods of plate.46 patients in Group A who were treated with expert tibial nail.43 patients in Group B were treated with MIPPO technology combined with LCP plate surgery.There are 89 patients.Postoperative follow-up of 3 to 12 months(average 9 months),by comparing the two treatments’ patient fracture classification,the operative time,the use of intraoperative fluoroscopy,the operative blood loss,time of fracture healing,the clinical,radiographic results,complications rates and clinical effect,we draw the result.The application of SPSS23.0 statistical software for statistical analysis of the results,α=0.05.level test.Results43 patients were treated with percutaneous locking plate,and 46 patients were treated with expert tibial nail.The results obtained from these two treatments were evaluated by comparing the operative time,the use of intraoperative fluoroscopy,the operative blood loss,time of fracture healing,the clinical,radiographic results,complications rates.Mean operation time for Group A and Group B were(62.59 ±5.516)minutes and(59.53 ± 5.400)minutes respectively(p=0.010).Mean use of intraoperative fluoroscopy was 8.35±3.027 in Group A,and 6.93±2.364 in Group B(p=0.016).Mean operation blood loss was(154.93±23.108)m1 in Group A and(148.60 ± 20.475)ml in Group B(p=0.176).Mean time of fracture healing was(16.24± 2.790)weeks in Group A and(17.74 ± 3.079)weeks in Group B(p=0.018).No significant difference was observed between Group A and Group B regarding ankle and knee joint function(p=0.758)and complications(p=0.69).There were statistical difference between the two groups in the operative time,the use of intraoperative fluoroscopy and time of fracture healing.Group B whose patients were treated by percutaneous locking compression plate has advantages in the operative time,the use of intraoperative fluoroscopy,and the group A treated by expert tibial nail has a shorter time of fracture healing.Besides,there was no statistically significant difference between the two groups in the operative blood loss,the clinical,radiographic results,complications rates and clinical effect.ConclusionsConsidering soft tissue complications and periosteal blood circulation,we recommend minimally invasive percutaneous plate osteosynthesis(MIPPO)technology combined with LCP internal fixation and expert tibial nail as the two main choices for treatment of the middle and distal tibial multiple fractures.Comparing these two methods shows that minimally invasive percutaneous plate osteosynthesis(MIPPO)technology combined with LCP internal fixation have more advantage in reducing the operation time and the use of intraoperative fluoroscopy.However,Expert tibial nial has a shorter healing time,which means MIPPO technology combined with LCP internal fixation is more simple and ETN has advantage in shortening the healing time.
