| Purpose:Using the basic theory of traditional Chinese medicine, combined with modern research, through the study of patients with cerebral small vessel disease,to explore the correlation between MRI imaging findings and TCM syndromes in patients with cerebral small vessel disease. It adds new content for the objectification and standardization of TCM syndrome differentiation and treatment, and provides an objective basis for TCM to better play a role in the prevention and treatment of cerebral small vessel disease.Method: From February 2018 to February 2019,538 patients from the Affiliated Hospital of Shandong University of Traditional Chinese Medicine and the Second Affiliated Hospital of Shandong University of Traditional Chinese Medicine met the diagnostic criteria for cerebral small vessel disease to explore correlation between MRI findings of cerebral small vessel disease and TCM syndromes.Statistical analysis using SPSS23.0 statistical software.Result: Among the imaging findings of 538 patients,538 were lacunar infarction,278 were white matter hyperintensities,94 were recent small subcortical infarct,260 were perivascular space, and 56 were cerebral microbleeds.Among them, there were 90 people with liver Yang, accounting for16.7%,35 people with wind phlegm obstruction, accounting for 25.1%,75 people with phlegm heat and bowel excess, accounting for 13.9%,174 people with qi deficiency and blood stasis, accounting for 32.3%,64 people with yin deficiency pneumatic, accounting for 11.9%.Among 538 patients with cerebral small vessel disease, they all had lacunar infarction;174 patients with qi deficiency and blood stasis had the highest proportion;64 patients with yin deficiency pneumatic had the lowest proportion. There were 278 patients with white matter hyperintensities, accounting for 52% of all patients;87 patients with qi deficiency and blood stasis, the highest proportion;37 patients with liver Yang, the lowest proportion. There were 94 cases of recent small subcortical infarct, accounting for 17% of all patients;among them,30 cases of wind phlegm obstruction were the highest, and the proportion of yin deficiency pneumatic was 7 cases, the lowest proportion. There were 56 cases of cerebral microbleeds, accounting for 10% of all patients;among them,24 patients of wind phlegm obstruction accounted for the highest proportion;7patients of liver yang, the lowest proportion. There were 260 patients with perivascular space, accounting for 48% of all patients;82 patients of qi and blood stasis had the highest proportion;33 patients of phlegm heat and bowel excess had the lowest proportion. There were differences in whether there were white matter hyperintensities, recent small subcortical infarct, and cerebral microbleeds in each syndrome type, and there was no difference in whether there were perivascular spaces. There were differences between the syndrome types and the grade of white matter hyperintensities、 recent small subcortical infarct and lacunar infarction and there was no difference between the syndrome types and the grade of cerebral microbleeds and perivascular space.Conclusion:1.Among 538 patients with CSVD,the incidence of qi deficiency and blood stasis was the highest, and the incidence of yin deficiency pneumatic was the lowest;2.There were differences in whether there were white matter hyperintensities, recent small subcortical infarct, and cerebral microbleeds in each syndrome type, and there was no difference in whether there were perivascular spaces.3.There were differences between the syndrome types andthe grade of white matter hyperintensities、recent small subcortical infarct and lacunar infarction and there was no difference between the syndrome types and the grade of cerebral microbleeds and perivascular space. |
