Intervention Study Of Exercise Prescription On Pulmonary Function,aerobic Capacity And Cancer-induced Fatigue In Patients With Non-small Cell Lung Cancer

Objective: Patients with lung cancer often face clinical problems such as impaired lung function,severe cancer-related fatigue,low immune level,lack of aerobic capacity and so on due to the progression of the disease itself and the side effects of a variety of treatments.We already have preliminary clinical evidence that exercise can improve the above problems to some extent.The purpose of this study was to develop an improved exercise prescription for patients with non-small cell lung cancer(combined with Qigong Baduanjin and respiratory training apparatus)to explore the effect of improved exercise prescription on improving lung function,cancer-related fatigue,immune level(NK cell activity)and aerobic capacity in patients with non-small cell lung cancer.Methods: In this study,66 patients with lung cancer were selected from the outpatient department of the Department of Respiratory Medicine of Cancer Hospital of Chinese Academy of Medical Sciences and the ward of Department of Respiratory Medicine of China-Japan Youhao Hospital.They were divided into control group(n =26)and experimental group 1(n = 26)and experimental group 2(n = 14)according to the order of admission or admission.The patients in the control group were given health education(out of ethical concern,the exercise prescription was instructed after the end of the experiment).On the basis of health education,the first group was given exercise prescription intervention(including resistance training and medium-intensity aerobic exercise combined with fitness Qigong Baduanjin,etc.),and group 2 added daily breathing training on the basis of this intervention.The lung function,cancer-related fatigue,NK cell activity and aerobic capacity of the two groups were evaluated before and after the experiment.The survey tools include: CFS cancer-related fatigue questionnaire,aerobic capacity evaluation(6-minute walking test combined with dyspnea score immediately after exercise).The test indexes included lung function(VC,FVC,FEV1)and NK cell activity.SPSS23.0 statistical software was used to analyze the data,and the statistical methods included descriptive statistics,independent sample T-test,repeated measurement analysis of variance and so on.Results:(1)The baseline indicators showed that patients with lung cancer had mild obstructive pulmonary dysfunction,moderate cancer-related fatigue,low NK cell activity and mildly decreased aerobic capacity.The baseline equilibrium of each index among the three groups was comparable(P > 0.05).(2)Before intervention,there was no significant difference in the total score of cancer-related fatigue and the summary score of each item among experimental group 1,group 2 and control group(P > 0.05).After intervention,the total scores of cancer-related fatigue,physical fatigue,emotional fatigue and cognitive fatigue in the experimental group were significantly lower than those in the control group(P < 0.05).After exercise prescription intervention,the total score of emotional fatigue and fatigue score of patients in the experimental group decreased significantly(P < 0.05).(3)There was no significant change in the activity of NK cells in the control group after intervention,but it was significantly increased in experimental group 1 and group 2 after intervention(P < 0.05).After intervention,the activity of NK cells in experimental group 1 was significantly higher than that in the blank control group(P < 0 05),but there was no significant difference in NK cell activity between experimental group 2and control group and experimental group 1.(4)Before intervention,there was no significant difference in 6-minute walking distance among experimental group 1,experimental group 2 and control group(P > 0.05).After intervention,the 6-minute walking distance of patients in experimental group 1 and 2 increased significantly(P< 0.05),and the score of dyspnea immediately after 6-minute walking was significantly lower than that before intervention(P < 0.05).However,there was no significant difference in 6-minute walking distance and dyspnea score between the control group and the control group(P > 0.05).(5)Before intervention,there was no significant difference in VC among experimental group 1,experimental group 2 and control group.After intervention,the VC,FVC and FEV1 of experimental group 2were significantly increased,and the FVC of experimental group 1 was significantly increased(P < 0 05),but compared with the control group,there was no significant change in VC,FVC and FEV1 of experimental group 1 and 2 after intervention.Conclusion:(1)After exercise intervention and exercise combined with respiratory training device intervention,the lung function,cancer-related fatigue,aerobic capacity and immune function of convalescent patients with lung cancer were improved.And the latter improved more dimensions of pulmonary function than the former.(2)Compared with the control group,the exercise prescription intervention significantly improved the total score of cancer-related fatigue,the score of emotional fatigue and the activity of NK cells in patients with lung cancer,but there was no significant improvement in lung function and aerobic capacity compared with the control group.
