Effect Of Cangfu Daotan Decoction On Adiponectin Receptor MRNA Expression And Pregnancy Outcome Of Phlegm-dampness PCOS Ovarian Granulosa Cells

Objective:To investigate the effect of Cangfu Guitang Decoction on the expression of adiponectin receptor gene(Adipo R1/Adipo R2 m RNA)in ovarian granulosa cells and the pregnancy outcome in patients with infertile polycystic ovary syndrome(PCOS),and to explore the mechanism of infertility in phlegm-dampness PCOS patients and the mechanism of Cangfu Daotan Decoction in improving abnormal lipid metabolism in PCOS patients.Methods:Sixty-six patients with polycystic ovary syndrome(PCOS)diagnosed as phlegm-dampness type receiving in vitro fertilization-embryo transfer(IVF-ET)were randomly divided into traditional Chinese medicine group(Cangfu Daotan Decoction +IVF)and placebo group(placebo + IVF),and 33 patients with non-phlegm-dampness type were selected as blank control group(placebo + IVF).All three groups adopted the dual-descent scheme of Daphelin+Diane-35.The basic clinical data of the three groups were observed.The levels of E2,LH and P on the day of HCG injection,the number of eggs obtained,fertilization rate,2PN cleavage rate,good embryo rate and clinical pregnancy rate were also observed in the three groups.Adiponectin receptor gene(Adipo R1/Adipo R2)expression in granulosa cells was measured by real-time fluorescence quantitative PCR(RT-PCR)and enzyme-linked immunosorbent assay(Elisa).Results:1.After treatment with Cangfu Daotan Decoction+IVF,the scores of phlegm-dampness syndrome and body fat percentage decreased before treatment,while the E2 level,egg number,fertilization rate,available embryo rate,clinical pregnancy rate and adiponectin concentration(APN)of HCG day were significantly higher than those of placebo group(placebo+IVF),the difference was statistically significant(P<0.05).2.The expression level of somatic gene(Adipo R1/Adipo R2)was lower in phlegm-dampness typethan in non-phlegm-dampness type(P < 0.05),higher in Chinese medicine group(Cangfu Daotan Decoction + IVF)than in placebo group(placebo + IVF),and the difference was significant(P < 0.05).3.Adiponectin receptor gene was negatively correlated with body fat percentage.Conclusion:1.Cangfu Daotan Decoction can significantly improve the clinical symptoms of phlegm-dampness and abnormal lipid metabolism in patients with phlegm-dampness PCOS infertility,improve ovarian function of storing and discharging,and ultimately improve the clinical pregnancy rate.2.Cangfu Daotan Decoction may improve the pregnancy outcome of PCOS patients by up-regulating the expression level of adiponectin receptor gene(Adipo R1/Adipo R2)and improving the percentage of fat in the body,compared with restoring the function of ovarian catharsis.3.Based on adiponectin receptor gene,the mechanism of phlegm-dampness infertility is explained,and the scientific connotation of the theory of drying dampness,resolving phlegm and promoting reproduction is enriched.
