Objective: To study the correlation between bone mineral density(BMD)and muscle mass,fat content,uric acid(UA)level and homocysteine(HCY)level in the elderly.Methods: A total of 200 patients aged 60 years or older who were admitted to the First Hospital of Lanzhou University from December 2018 to December 2021 were collected.The subjects were divided into three groups based on BMD: normal bone mass group,osteopenia group and osteoporosis group(OP).The age,sex,body mass index(BMI),total fat content(TFM),body fat rate,appendicular skeletal muscle mass(ASM),relative skeletal mass index(RASM),total muscle mass(TLM),hemo globin(Hb),UA,HC Y,albumin(ALB),blood glucose(Glu),globulin(GLB),calcium(Ca),phosphorus(IP),blood lipid and other indicators of the three groups were compared.At the same time,the correlation between BMD and age,BMI,ASM、TLM、RASM、TFM、body fat rate、UA、HCY、ALB、GLB、Hb、total cholesterol(TC)、high density lipoprotein cholesterol(HDL-C)and Glu was analyzed,and the independent related factors of bone mineral density were found by multiple linear regression analysis.Results:1.There were 131 elderly females and 69 elderly males in the study population.There were 75,68 and 57 persons with normal bone mass,osteopenia,osteoporosis respectively,accounting for 37.5%,34% and 28.5% of the total number,respectively.2.The incidence of osteoporosis was 26.83% in normal RASM group and 31.17%in low RASM group,but there was no significant difference(P>0.05).The incidence of osteoporosis in different age groups was 19.51%,34.52% and 35.29%,respectively,with no statistical significance(P>0.05);but the incidence of low RASM in different age groups was 36.59%,28.57%,67.65%,respectively,and some of the differences were statistically significant(P<0.05).The incidence of osteoporosis in female was higher than that in male(41.22% vs 4.35%),while the incidence of low RASM in female was lower than that in male(32.06% vs 50.72%),which the differences were both statistically significant(P<0.05).The RASM、TFM、total body BMD and L1-4BMD of super recombinant group were higher than those of normal weight group,while only RASM and TFM in obese group were higher than those in normal weight group,the differences were statistically significant(P<0.05).There was no significant differences in the total body BMD、total body T、L1-4 BMD and L1-4 T between physiological concentration UA group and hyperuricemia group(P > 0.05).3.The levels of BMI,ASM,RASM,TLM,Hb,alanine aminotransferase(ALT)and Glu in osteopenia group were lower than those in normal bone mass group,while the levels of age,GLB and HDL-C were higher than those in normal bone mass group,the differences were statistically significant(P<0.05).The levels of BMI,ASM,RASM,TLM,TFM,Hb and UA in osteoporosis group were lower than those in normal bone mass group,while the levels of age,GLB,TC and HDL-C were higher than those in normal bone mass group,the differences were statistically significant(P<0.05).4.Systemic BMDand L1-4 BMD were linearly and negatively correlated with age and HDL-C,and linearly and positively correlated with BMI,RASM,ASM,TLM,TF M,UA,Hb,ALT,GLB and Glu(P<0.01 or P<0.05),but not with HC Y,TC,GLB,and ALB(P>0.05).TLM was linearly and negatively correlated with age and linearly and positively correlated with BMI,TFM and UA(P<0.01 or P<0.05).TFM was linearly and positively correlated with BMI and UA(P<0.01 or P<0.05).5.Age,sex,RASM,UA and Glu were independently correlated with systemic BMD(P<0.05).Gender,RASM,body fat rate and Glu were independently correlated with L1-4 BMD(P< 0.05).Conclusion:1.BMD in the elderly was negatively correlated with age and HDL-C,and positively correlated with BMI,muscle mass,fat content,UA,Glu and Hb,but not HCY.2.RASM,sex and Glu were independently associated with systemic BMD and L1-4 BMD,while age and UA were only independently associated with systemic BMD,and body fat rate was only independently associated with L1-4 BMD.
