Objective:To analyze the correlation between skeletal muscle mass,fat content and bone mineral density in postmenopausal type 2 diabetes mellitus(T2DM)women,so as to provide reference for early detection and early prevention of osteoporosis.Research objects and methods:Retrospective analysis was performed on postmenopausal female patients hospitalized and out-patient in the endocrinology Department of Yanbian Hospital from January 2018 to January 2020.A total of 99 patients met the inclusion criteria,including 54 patients with T2DM in the diabetes group,with an average age of 60.9 years.The control group consisted of 45 patients without diabetes,with an average age of 59.8 years.Collect the patient’s general information,disease history,smoking and drinking history,medication history,etc;measure the height,weight and blood pressure at admission.The test items included blood glucose index,blood lipid index,renal function related indexes,lumbar 11-L4 bone mineral density,right femoral neck bone mineral density and all bone mineral density of right femur,visceral fat area,body fat volume,skeletal muscle mass and right lower limb muscle mass.SPSS22.0 software was used for statistical analysis to compare whether there were statistically significant differences in BMD values,biochemical indicators,skeletal muscle mass,right lower limb muscle mass,visceral fat area and body fat mass between the two groups,and analyze the correlation between skeletal muscle mass,fat content level and bone mineral density.Then,all the subjects were divided into three groups according to their bone mineral density.Among them,31 cases were in the osteoporosis group,with an average age of 60.8 years.There were 34 patients in the group with decreased bone mass(mean age:60.4 years).There were 34 patients in the normal BMD group,with an average age of 60.1 years.The differences of biochemical indexes,visceral fat area,body fat,skeletal muscle mass and right lower limb muscle mass among the three groups were compared to observe whether the differences were statistically significant.Results:1.Fasting blood glucose,visceral fat area and fat mass in the diabetes group were all higher than those in the control group,while high-density lipoprotein cholesterol,lean mass and right lower limb muscle mass were all lower than those in the control group,with significant differences(P<0.05).There were no significant differences between the two groups in general conditions such as height and weight,TC、LDL-c and bone mineral density.2.Pearson correlation analysis found that the local BMD value of postmenopausal women in the diabetes group was statistically correlated with fat mass,lean mass,right lower limb muscle mass and visceral fat area(P<0.05).In the control group,the local BMD value of postmenopausal women was statistically correlated with body fat mass,lean mass and right lower limb muscle mass(P<0.05).The multiple stepwise regression analysis of the factors with statistical association after the above correlation analysis showed that skeletal muscle mass was a significant influencing factor of 11-14,right femoral neck and right femoral BMD in postmenopausal women with T2DM.3.All the subjects were divided into osteoporosis group,bone mass reduction group,bone density normal group,height,weight and other general conditions and blood biochemical indicators(FBG,TG,TC,HDL-C,LDL-C,BUN,CRE,CYS-C)between the three groups showed no statistical significance(P>0.05).The levels of skeletal muscle mass and body fat mass in the osteoporosis group were lower than those in the normal and reduced BMD groups,and the differences were statistically significant(P<0.05).Conclusion:1.Postmenopausal T2DM women have decreased skeletal muscle mass and increased fat content.2.There was a statistical correlation between skeletal muscle mass and fat content and local BMD in postmenopausal women with T2DM,and the level of skeletal muscle mass was closely related to L1-L4,right femoral neck and right femoral BMD.
