| Objective: Frailty is the most common type of geriatric syndromes.It is considered to be the early manifestation of disability and dysfunction,which can predict the adverse health outcomes of the elderly to a certain extent.Studies have shown that frailty elderly are more likely to have adverse events such as falls,fractures,delirium,disability,hospitalization and even death,which affect the quality of life of the elderly in their later years.On the one hand,based on the understanding of related factors of frailty,this study will screen out risk factors that are highly related to frailty,so as to have a relatively accurate grasp of frailty from a deeper level,and provide a basis for precision rehabilitation interventions;Another,starting from the risk factors that are highly related to frailty,this study will explore the effect of task-oriented training based on PNF theory on frail elderly patients,to optimize rehabilitation programs and provide a more convenient and effective rehabilitation treatment method for frail elderly patients.Methods: In this study,117 inpatients in the Department of Geriatrics of Xi’an Hospital of Traditional Chinese Medicine were selected.A total of 106 patients met the inclusion criteria and were the subjects of the first part of the study.They were assessed for frailty(Fried score),their basic information(including age,Gender,education level,height,weight,whether living alone)and laboratory indicators(including blood routine,liver function,kidney function,and blood lipids)were collected,and performed scales to assess the patients’ ability such as instrumental activities of daily living(Lawton-Brody IADL score),Short-Physical Performance Battery(SPPB),depression screening scale score(PHQ-9),Mini-nutritional assessment(MNA),Athens Insomnia Scale.Pearson correlation analysis between the selected index and the frailty score(Fried score)were used to screened out indicators highly related to Fried scores,which are used as independent variables,Fried scores are used as dependent variables for multiple stepwise regression analysis.(2)Among the above-mentioned patients,20 patients with Fried score ≥1 were selected,and they were randomly divided into 2 groups,10 in the experimental group and 10 in the control group.The experimental group was given task-oriented training based on PNF theory for 8 weeks,the control group carried out general activities of daily living,and meatured the grip strength,4m walking timeand time up and go test(TUGT)of the two groups of patients before and after the intervention.The independent sample t test was used to analyze the evaluation of the two groups of patients,and the paired sample t test was used to analyze each the indicators of the group before and after intervention.Results:(1)In this study,Pearson correlation analysis results showed that age,grip strength,BMI,SPPB,Lawton-Brody IADL score,PHQ-9,MNA,red blood cell count,albumin,alanine aminotransferase(ALT)were significantly correlated with Fried score(P< 0.05);The results of multiple linear regression analysis showed that grip strength,SPPB and MNA were independent influence factors of frailty(P<0.05).(2)The effect of task-oriented intervention based on PNF theory: there was no significant differences between the two groups before the intervention in grip strength,4m walking time and TUGT(P>0.05).After 8 weeks of intervention,the two groups had no significant differences in grip strength,4m walking time and TUGT(P>0.05),but the difference before and after the intervention between the two groups and the comparison within the experimental group before and after the intervention were significantly different or very significantly different(P<0.05 or P<0.01),the grip strength was significantly improved(P<0.05),the 4m walking time and TUGT were shorter(P<0.05),while the control group before and after the intervention had no significant difference in internal comparison(P>0.05).Conclusion:(1)In this study,factors that are significantly related to frailty are: age,grip strength,BMI,lower limb physical performance,instrumental activities of daily living,depression,nutrition,red blood cell count,albumin level,alanine aminotransferase level,among which grip strength,lower limb physical performance and nutrition are independent influence factors of frailty.(2)An eight-week task-oriented training based on PNF theory can improve the motor function of frail patients,such as grip strength,pace and balance ability. |
