Objective: To compare the clinical effects of propofol combined with butorphanol or remifentanil and propofol alone for painless induced abortion anesthesia,and to find the best anesthesia method to provide the basis for painless induced abortion anesthesia.Methods: A total of 120 patients with painless induced abortion in Shengjing Hospital of China Medical University were randomly separated into three groups: group C(control group,simple propofol),group B(butorphanol plus propofol)and group R(remifentanil plus propofol).Analgesia,injection pain and the dosage of propofol,recovery time,intraoperative heart rate,blood pressure,finger oxygen saturation and other hemodynamic changes were surveyed and recorded in the three groups.Intraoperative limb movement,respiratory depression,postoperative nausea and vomiting,uterine contraction pain and the incidence of pain and other adverse reactions were also observed and recorded.Results: The differences between age,weight,BMI and other general conditions are not of significance among the three groups.The total dosage and additional times of propofol in group C were significantly more than those in groups B and R,and the recovery time was prolonged,and the difference had statistical significance(P < 0.05);the incidence rate of injection pain in groups B and R was significantly lower than that in group C,and the difference had statistical significance;the intraoperative hemodynamic fluctuation in groups B and R was more stable than that in group C,and the vital signs were more stable;the incidence rate of hypotension,bradycardia,respiratory depression,limb movement and other adverse reactions in groups B and R was lower than that in group C,and the difference had statistical significance;the incidence rate of postoperative uterine contraction pain and the degree of pain in the three groups: group B < group R < group C,and the difference had statistical significance(p < 0.05).Conclusion:In painless induced abortion anesthesia,propofol combined with butorphanol or remifentanil both are superior to propofol alone in the anesthetic effect,and the incidence of hypotension,bradycardia,respiratory depression and other adverse reactions in group B is significantly lower than group C.Group B is significantly superior to group R in reducing the degree of postoperative uterine pain,and can significantly reduce the injection pain and maintain the basic vital signs of blood pressure,heart rate in patients during surgery,thus,low-dose butorphanol can also be safely suitable for short day surgery.
