Surgical induced abortion is a common remedy for contraceptive failure,usually processed under intravenous anesthesia in clinical practice.Uterine contraction pain often occurs after operation and can affect recovery or induce more severe complications among women.To release this kind of pain,we performed a randomized controlled trail using common analgesics including fentanyl and butorphanol from January to Octorber in 2016.Ninety nine patients meeting our inclusion criteria were invited to our study and were randomly divided into three groups: propofol,fentanyl and butorphanol group.In propofol group,patients were anesthetized with single use of propofol,while in fentanyl group,combination of fentanyl and propofol were adminstered,so as the butorphanol group.Basic information,evaluation of anesthesia effect,changes of vital signs,incidence of complications and Visual Analogue Scale(VAS)score for uterine contraction pain were collected and analyzed.Results showed that butorphanol and fentanyl group demonstrated superior anesthesia effect than propofol group(P<0.001)and no significant differences were found between the former two groups(P=0.064).Butorphanol group consumed least propofol than the other two groups(115.91±21.70 mg,P<0.01),and the recovery time(14.00±1.35 min)was a bit longer than fentanyl(P=0.001)and propofol group(12.61±1.91,P=0.2416)although difference was not significant.Hypotension in fentanyl group was obvious and the incidence(48.5%)was higher than propofol(P=0.009)and butorphanol group(30.3%,P=0.131).Incidence of dizziness was also higher in fentanyl group compared with the other two groups(P=0.007),while drowsiness occurred most frequently in butorphanol group(P=0.000).Changes of vital signs were greatest in fentanyl group and minimal in propofol group.Uterine contraction pain after surgery was most severe in propofol group(P=0.003)and was slightest in butorphanol group(P<0.01)when discharged.Our study provided evidence for selecting proper analgesics in induced abortion,butorphanol was more suitable on stabilizing vital signs and preventing uterine contraction pain than fentanyl during and after operation. |
