Objective: To observe the relationship between the efficacy of anti-VEGF treatment of different types of diabetic macular edema(DME)and the structural changes of the macular area.Methods: A total of 39 eyes(29 patients)who were diagnosed with diabetic macular edema in the Department of Ophthalmology,*** Hospital Affiliated to *** from January2020 to January 2021 and were followed up for 2 weeks after anti-VEGF treatment were retrospectively included in the study.According to the OCT scan results,they were divided into 15 eyes in DRT group,14 eyes in CME group,and 10 eyes in SRD group.All patients underwent BCVA and CRT baseline measurements before and after treatment.GCC thickness,FAZ area,SCP and DCP blood flow density were evaluated at the same time,and the effect of anti-VEGF treatment on the changes in macular structure of patients in different groups was analyzed.Results: 1.At the follow-up 2 weeks after treatment,the CRT of the three groups was significantly lower than before treatment(P<0.05),but the difference between the groups was not statistically significant(P>0.05);during the follow-up,the DRT group and CME showed There was a statistically significant difference compared to before treatment(P<0.05).Although the SRD group had a decrease compared to before treatment,the difference was not statistically significant.There was no significant statistically significant difference between the three groups(P>0.05).2.GCC of patients with different types of DME was significantly reduced after anti-VEGF treatment(P<0.05).There was no significant statistical difference in GCC(μm)levels between groups(P>0.05);3.The blood flow density of DCP in the SRD group was significantly lower than that in the CME and DRT groups,while the DCP blood flow density between CME and DRT was not statistically significant.4.Analyze the correlation of each index: GCC is positively correlated with CRT(P<0.05),negatively correlated with FAZ(P<0.05),and CRT is negatively correlated with FAZ(P<0.05).Conclusions: The thickness of the retina in patients with DRT diabetic macular edema decreases more significantly after anti-VEGF treatment.Generally,the changes in the retinal nerve structure in DME patients are related to the changes in microvessels.The blood flow density of the deep capillary plexus(DCP)may have an effect on the anti-VEGF curative effect of patients,which affects the anti-VEGF curative effect of DME through OCT examination.The analysis of OCT and OCTA characteristics before treatment is positive to guide the patient’s drug choice significance.
