Objective: To investigate the relationship between obesity and the occurrence of papillary thyroid carcinoma(PTC)。Methods: The clinical data of 198 patients with thyroid nodules treated by surgery were collected.According to the postoperative pathology,the patients were divided into benign thyroid nodule(BTN)group(n = 72)and papillary thyroid carcinoma(PTC)group(n = 126).According to the characteristics of lymph node metastasis,papillary thyroid carcinoma was divided into three groups: papillary thyroid carcinoma without lymph node metastasis group A(n = 68),papillary thyroid carcinoma with central lymph node metastasis group B(n = 37)and papillary thyroid carcinoma with lateral cervical lymph node metastasis group C(n = 21).According to the body mass index(BMI),the patients were divided into normal weight group,overweight group and obesity group,and then central obesity group and non-central obesity group were defined according to waist circumference.Results: The independent sample T-test showed that the occurrence of papillary thyroid cancer was not related to serum anti thyroglobulin antibody(TG-Ab),total cholesterol(TC),adiponectin(ADPN)levels and systolic blood pressure(P > 0.05),but was related to age,fasting blood glucose,serum thyrotropin(TSH),anti thyroid peroxidase antibody(TPO-Ab),triglyceride(TG),high density lipoprotein(HDL),leptin(LEP),tumor necrosis factor-α(TNF-α)and diastolic blood pressure(P < 0.05).Chi square test results showed that obese patients with papillary thyroid cancer were significantly more than normal weight patients with papillary thyroid cancer(86.0% vs14.0%).Binary logistic regression analysis showed that HDL was the protective factor of papillary thyroid carcinoma(P < 0.05).BMI,diastolic blood pressure and serum TG level were the independent risk factors of papillary thyroid carcinoma(P < 0.05).Conclusions: The incidence of thyroid papilloma cancer increased significantly in obese patients.In obese patients,the metabolic disorder of adipokines,especially leptin and tumor necrosis factors-α,Stimulating the abnormal proliferation of thyroid cells and inhibiting their apoptosis lead to a significantly increased risk of papillary thyroid cancer.In addition,the incidence rate of papillary thyroid carcinoma was negatively correlated with HDL level,and positively correlated with diastolic blood pressure and triglyceride.
