Objective:(1)The differences of age,sex,BMI,TSH,TPO AB and diameter of thyroid nodules between patients with thyroid papillary carcinoma(PTC)and benign thyroid nodule(BTN)were analyzed retrospectively to explore the correlation between BMI and risk of PTC.(2)The differences of age,gender,TSH,TPO AB,thyroid nodule diameter between PTC and BTN patients were observed,and potential risk factors were found.Methodology:(1)The clinical data of 848 patients with thyroid diseases(486 in BTN group and 362 in PTC group)who were admitted to the Second Affiliated Hospital of Nanchang University and operated from June 2018 to July 2019 were collected.(2)The clinical data of sex,age,height,weight,TSH,TPO AB,thyroid nodule diameter were recorded in all patients.The BMI was calculated by weight(kg)/height(m~2).According to the guidelines for the control of overweight and obesity in Chinese adults,the patients were divided into normal weight group(BMI 18.5-23.9kg/m~2),super recombination group(BMI 24.0-27.9kg/m~2)and obesity group(BMI≥28kg/m~2).(3)According to the pathological results,they were divided into PTC group and BTN group.The differences of PTC and BTN in age,gender,TSH,TPO AB,thyroid nodule diameter and the proportion of PTC in BMI group were compared.(4)According to gender stratification,the differences of age,TSH,TPO AB,thyroid nodule diameter and other indicators between PTC and BTN patients,as well as the proportion of PCT in each BMI group were compared.(5)the incidence of PTC in different age groups was compared.(6)The influencing factors of PTC were analyzed by binary logistic regression.Results:(1)The age of PTC group was significantly lower than that of BTN group(44.85±11.210 years vs 52.88±11.066 years,p<0.001);there was a difference in age distribution between the two groups(p<0.001);the proportion of men in PTC group was significantly higher than that in BTN group(34.8%vs 20.4%,p<0.01);the mean BMI in PTC group was significantly higher than that in BTN group(24.925±3.0911)kg/㎡vs(23.761±3.0419)kg/㎡,p<0.001);the proportion of TPO AB positive in PTC group was significantly higher than that in BTN group(24%vs 9.1%,p<0.01);the nodule diameter in PTC group was significantly smaller than that in BTN group(1.483±0.9904)cm vs(3.022±1.4689)cm,p<0.001);the serum TSH level in PTC group was higher than that in BTN group,but the difference was not statistically significant(1.46(0.87,2.14)mIU/L vs 1.41(0.90,2.20)mIU/L,p>0.05);Compared with normal weight group,the proportion of PTC in overweight and obesity group was higher(p<0.05),and there was no significant difference between overweight and obesity group(p>0.05).(2)In male patients,the incidence of PTC in overweight and obesity group was higher than that in normal weight group(p<0.05),but in female patients,there was no significant difference(p>0.05).(3)According to the age and gender stratification,the incidence of PTC in the41-64 age group,overweight and obesity group was higher than that in the normal weight group(p<0.05),and there was no significant difference in other age groups(p>0.05);there was no significant difference in the incidence of PTC in different BMI levels in different age groups of female patients(p>0.05).(4)The Multivariate Logistic regression analysis showed that overweight and obesity,low age,male,TPO AB positive were independent risk factors for PTC,and nodule diameter was an independent protective factor for PTC.Conclusions:(1)BMI is an independent risk factor for PTC.The risk of PTC in overweight and obese people is higher than that in overweight people,and the risk ratio is higher in obese people.(2)Male is an independent risk factor for PTC,and the proportion of PTC in male patients with high BMI is high.(3)Low age is an independent risk factor for PTC,and the proportion of PTC in men aged 41-64 with high BMI is high.(4)TPO AB positive is an independent risk factor for PTC,Nodule diameter is an independent protective factor for PTC.
