Objective: To research and analyze the correlation between serum TSH receptor antibody(TRAb)level and leukocyte parameters,thyroid function in patients with Graves’ disease(GD).Methods: There were 353 GD patients participating in this study.They were divided into two groups using1.75U/L as TRAb’s threshold: TRAb-negative group(n=41)and TRAb-positive group(n=312),the differences in baseline data,leukocyte parameters and thyroid function between them were compared;Afterwards,these patients were regrouped according to gender,the differences in baseline data and laboratory test outcomes of the two teams were compared;Then these patients were redivided using 40 as age’s threshold: < 40 years old group(n=141)and ≥40 years old group(n=212),the differences in baseline data and laboratory test outcomes of these two teams were compared.Afrer that,the analysis of correlation was used to search the relationship between serum TRAb and baseline data,white blood cell parameters,thyroid function;Then the relationship between age and thyroid function was analyzed in the same way.Finally,in order to determine the influencing factors of serum TRAb,multiple linear regression was used.Results: 1.Comparison according to TRAb level:(1)Baseline data: There were more smokers in the TRAb-positive group than those in the TRAb-negative group(25.3% vs.4.8%,P < 0.05).(2)Leukocyte parameters: white blood cell count(WBC),nutrophil count(NEUT),NEUT%,neutrophil to lymphocyte ratio(NLR),monocyte to lymphocyte ratio(MLR)of TRAb-negative group were higher than those of TRAb positive group(P < 0.05),while lymphocyte count(LYMPH)and LYMPH% of TRAb-negative group were lower than those of TRAb-positive group(P < 0.05).(3)Thyroid function: Compared with the TRAb-positive group,the TRAb-negative group had higher level of TSH,while the levels of FT3,FT4,TT3,TT4,and TT3/TT4 were relatively lower(both P < 0.05).2.Comparison according to gender:Compared with the female group,male group’s smokers were relatively more(P < 0.01),the level of TSH in male group was lower,while levels of FT3,FT4,TT3,TT4,TT3/TT4 were higher(P < 0.05).3.Comparison according to age: there was no significant difference in each data and indicator between these two groups(P > 0.05).4.Correlation analysis:(1)TRAb and baseline data: TRAb was negatively correlated with the course(r=-0.118,P=0.027),medication history(r=-0.208,P < 0.001)and duration of medication(r=-0.150,P=0.006),while positively correlated with smoking history(r=0.123,P=0.020).(2)TRAb and leukocyte parameters: TRAb was negatively correlated with NEUT(r =-0.134,P=0.012),NEUT %(r=-0.197,P < 0.001),NLR(r=-0.291,P < 0.001)while positively correlated with LYMPH(r =0.115,P=0.031)and LYMPH%(r =0.202,P < 0.001).(3)TRAb and thyroid function: TRAb was negatively correlated with TSH(r=-0.541,P<0.001),while positively correlated with FT3(r=0.521,P<0.001),FT4(r=0.366,P < 0.001),TT3(r=0.517,P < 0.001),TT4(r=0.351,P < 0.001),thyroperoxidase antibodies(TPOAb)(r=0.184,P=0.001),thyroglobulin antibodies(TGAb)(r=0.123,P=0.021),TT3/TT4(r=0.346,P<0.001).(4)Age and thyroid function: Age was negatively correlated with FT3(r=-0.124,P=0.021).5.Multiple linear regression analysis showed that TSH was the negative influencing factor of TRAb(β=-0.975,P < 0.001),while FT3(β=0.369,P < 0.001)and TPOAb(β=0.010,P=0.001)were the positive influencing factors.Conclusions: 1.TRAb was negatively correlated with NEUT,NEUT % and NLR,while positively correlated with LYMPH and LYMPH%.2.Serum TRAb level of GD patients was negatively correlated with the course of disease,medication history and duration of medication,while positively correlated with smoking history.3.There were differences in smoking history,WBC,NEUT,NEUT%,NLR,MLR,and thyroid function between TRAb-negative group and TRAb-positive group. |
