Objective:The aim of this study is to investigate the serum 25-hydroxy vitamin D level and the distribution of in patients with type 2 diabetes mellitus,to discuss the relationship between serum 25-hydroxy vitamin D level and diabetic macrovascular disease, and to identify the risk factors of diabetic macrovascular disease. Methods:A total of 100 patients with type 2 diabetes were enrolled in this study from February 2019 to February 2020.They were divided into type 2 diabetes without macrovascular disease(T2DM,n=50)group and type 2 diabetes with macrovascular disease(T2DM+MVD,n=50)group.Clinical characteristics were collected.25-(OH)-D,HbA1c,CRP,TC,TG,LDL-c,HDL-c were measured.SPSS 25.0 was performed for statistical analysis.Statistical significance between two groups was evaluated using t test.Logistic regression was used to identify the risk factors of diabetic macrovascular disease.Results:1.The clinical characteristics of these patientsCompared with T2DM group, the age of T2DM+MVD group was significantly older. The proportion of diabetes family history in patient with T2DM+MVD was higher than that in T2DM group(P<0.05).There was no significant difference in BMI and gender between two groups(P>0.05).2.The biochemical characteristics between two groupsCompared with patients with T2DM group, the serum level of 25-(OH)-D and HDL-c were significantly decreased while HbA1c,TC,LDL-c were significantly increased in patients with T2DM+MVD group(P all<0.05).There was no significant difference in serum CRP and TG levels between two groups(P>0.05).3.The distribution of serum 25-(OH)-D between two groupsCompared to T2DM group, the proportion of patients with vitamin D inadequate/deficiency was significantly higher in patients with T2DM+MVD group(χ~2=29.27,P<0.05).4.The correlation between 25-(OH)-D and lipid profile,HbA1cThere was negative correlation between serum 25-(OH)-D and TC(P=0.001,R=-0.33),TG(P=0.018,R=-0.24),LDL-c(P=0.011,R=-0.25),HbA1c(P=0.001,R=-0.32).There was no correlation between serum25-(OH)-D and HDL-C(P=0.599,R=0.05).5.The risk factors of diabetic macrovascular diseaseLogistic regression analysis was performed to check the risk factors of diabetic MVD.Diabetic MVD was used as the dependent variable while age, serum 25-(OH)-D,TC,LDL-c,HbA1c and family history were used as independent variables. The results showed that, lower serum 25-(OH)-D(P=0.000,OR=0.72)and HbA1c(P=0.031,OR=1.47)were risk factors of diabetic MVD.Conclusions:1.The serum 25-(OH)-D level is lower in patients with T2DM+MVD,vitamin D inadequate/deficiency is more dominant in patients with T2DM+MVD.2.Serum 25-(OH)-D is negative correlate with TC,TG,LDL-c,and HbA1c.3.Lower serum 25-(OH)-D and HbA1c are the risk factors of diabetic MVD. |
