objective: To evaluate the role of the Smart Disease Management System(SSDM)for implementing disease management in patients with rheumatoid arthritis(RA).methods: Patients who attended the outpatient clinic of the Department of Integrative Medicine of the First Hospital of Chongqing Medical University from June 2016 to December 2019 who met the diagnosis of RA used the SSDM Self-Assessment Disease Activity Index(DAS28),Stanford Health Assessment Questionnaire(HAQ),Time to Morning Stiffness,Clinical’s Disease Activity Index(CDAI),and Simplified Disease Activity Index(SDAI)scales.Time was assessed at baseline,second and third time,respectively,and the data were statistically analyzed using SPSS24.0 software.Results: All variables were higher in patients who did not meet the standard at baseline than in those who did,and the disease activity of patients was positively correlated with age,number of painful joints,number of swollen joints,blood sedimentation,C-reactive protein,time to morning stiffness,patient self-assessment value,HAQ,SDAI,and CDAI,and C-reactive protein,number of painful joints,and blood sedimentation had significant effects on the change of disease activity in RA patients.The difference of each index was not statistically significant,but each index improved significantly after the second and third assessment compared with baseline,and the proportion of moderate disease activity and high disease activity were significantly lower after the second and third assessment compared with baseline,and the attainment rate was significantly higher compared with baseline(P < 0.05).Conclusion: Patients with RA can self-manage well through SSDM,keep abreast of their condition,follow up regularly,improve compliance,and achieve better health outcomes.the SSDM chronic disease management model can significantly reduce patients’ disease activity,improve somatic function,and increase the attainment rate of patients with RA.
