The Effect Of Empagliflozin On The Urinary Protein Of Diabetic Nephropathy

Objective:To observe the effect of empagliflozin on the urinary protein in of diabetic nephropathy patients.Methods:150 patients with diabetic nephropathy were randomly divided into experimental group and control group,Control group: maintain the original hypoglycemic program.Experimental group: the original hypoglycemic regimen was added with empagliflozin(10 mg,1 tablets / day),and the blood glucose control in the two groups was not up to standard with insulin adjustment.The two groups were treated for 90 days,Follow-up at 30 days、60 days、90 days,before and after treatment.blood pressure、Waistline、Weight、BMI、blood fat、blood glucose、UA、IL-6、urine protein ecorded each time,Glycosylated hemoglobin c(Hb A1c)was measured before and at 90 days.Record adverse reactions.Results:A total of 145 patients completed the experiment.There was no significant difference in the baseline value between the two groups(P>0.05).here was significant difference in SBP、Waistline、Weight、BMI、TG、FBG、UA、ACR、IL-6、Hb A1c、24 h UP between experimental group and control group after 90 days of treatment(P<0.05).There was no significant difference in DBP、TCHO、LDL-C、e GFR(P>0.05).There were significant differences in SBP、DBP、Waistline、weight、BMI、TG、FBG、UA、ACR、IL-6、Hb A1c、24h UP before and after 90 days of treatment(P<0.05).There were significant differences SBP、DBP、weight、TG、UA、e GFR 、 ACR 、 IL-6 、 24 h UPin the experimental group 30 days after treatment(P<0.05),and there were significant differences in Waistline 、Weight、BMI、TG、FBG、UA、ACR、IL-6、24 h UP in the experimental group after 60 days of treatment(P<0.05).The experimental group and the control group were analyzed by repeated measurement of variance,and the time effect: the change trend of waist circumference,weight and BMI、TG、FBG、UA、e GFR、ACR、IL-6、24h UP was statistically significant SBP、DBP、the two groups with the prolongation of time(P<0.05).the inter-group effect: the difference between the two groups in SBP、DBP、weight and BMI、TG、FBG、UA、e GFR、ACR、IL-6、24h UP was statistically significant.Interaction effect: The difference of body weight and BMI、TG、FBG、UA、e GFR 、 ACR 、 IL-6 、 24 h UP between the two groups was statistically significant(P<0.05).Conclusion:1.For patients with early diabetic nephropathy,empagliflozin had obvious effect of reducing urinary protein.2.the combination of empagliflozin and irbesartan had better effect than irbesartan alone.3.Empagliflozin can effectively reduce blood pressure,body weight,inflammatory response in patients with diabetic nephropathy.
