Study On Disease Burden Of Major Chronic Diseases In Shanxi Province

Objective:This study investigated the prevalence of chronic disease,smoking,drinking and other behavioral lifestyle of residents over 15 years old in Shanxi Province to understand the prevalence of major chronic disease in Shanxi Province.Combined with the monitoring data of death causes,disability adjusted of life years was used as the index to evaluate the disease burden caused by major chronic diseases and their related risk factor.The results could provide scientific basis for the formulation of chronic disease prevention and control strategies.Methods:This study used disability adjusted of life years to calculate the health loss of diseases,which included years of life lost and years of life lived with disability.The cause of death data comes from Chinese Death Cause Monitoring Data Collection.The prevalence of chronic diseases adopts the multi-stage random sampling method to conduct on-site surveys of residents aged 15 and over in 11 cities in Shanxi Province.The data were sorted out by Excel 2016 and analyzed by SPSS24.0.Descriptive analysis was used for general data and the comparison between different rates uses χ2test.The disease burden of 4 main chronic diseases attributed to smoking and drinking behavior was analyzed by counterfactual analysis.Results:1.The prevalence of chronic diseases in Shanxi Province was 48.80%.The top 5chronic diseases among city residents were hypertension(23.19%),diabetes(10.44%),coronary heart disease(4.75%),stroke(2.77%)and Osteoporosis(2.26%).The top five chronic diseases among rural residents were hypertension(28.90%),diabetes(9.84%),rheumatoid arthritis(6.60%),coronary heart disease(3.31%)and chronic gastroduodenal ulcer(2.84%).Gender,age,marital status and education level were the influencing factors of chronic diseases.2.In 2019,disability adjusted of life years due to 4 major chronic diseases was938,100 in Shanxi.In its composition,the years of life lost of stroke accounted for74.86%;the years of life lived with disability of diabetes and coronary heart disease accounted for 68.13% and 54.00% respectively.The DALY per thousand population caused by four chronic disease was 26.26,from high to low were coronary heart disease(49.28%),stroke(37.62%),diabetes(9.25%)and hypertension(3.85%).In terms of age,the disease burden per thousand population of the four diseases increased gradually with age.The disability adjusted of life years of diabetes and coronary heart disease was the highest in 45-59 years old group,stroke was the highest in 60-69 years old group,and hypertension was the highest in over 80 years old group.In terms of gender,the disability adjusted of life years per thousand population of hypertension,coronary heart disease and stroke in men was higher than that in women,and the disability adjusted of life years per thousand population of diabetes in women was higher than that in men.3.In Shanxi Province,the disability adjusted of life years of four chronic diseases caused by smoking was 194,700 in 2019,years of life lost was 110,100(56.55%),and years of life lived with disability was 84,600(43.45%).The disease burden of Coronary heart disease attributed to smoking was the heaviest,followed by stroke,hypertension,and diabetes.The disability adjusted of life years of three chronic diseases caused by drinking was 48,100 in 2017,years of life lost was 30,900(64.24%),and years of life lived with disability was 17,200(35.76%).The disease burden of Stroke attributed to drinking was the heaviest,followed by coronary heart disease and diabetes.In terms of gender,the disease burden caused by smoking and drinking was higher in men than in women.In terms of age,the disease burden caused by smoking and drinking in the 30-60 age group showed an overall upward trend,and began to decline over the age of 60.Conclusions:1.The prevalence of chronic diseases in Shanxi Province was still severe,with high prevalence rates of hypertension and diabetes.There were significant differences in the prevalence of chronic diseases among residents with different demographic characteristics.2.The disease burden of major chronic diseases in Shanxi province is large,from high to low is coronary heart disease,stroke,diabetes,hypertension,so it is necessary to focus on the prevention and control of coronary heart disease and stroke.At the same time,we should pay attention to the changes of DALY loss in different gender and age groups caused by behavioral risk factors,to formulate targeted prevention and control strategies.3.The disease burden of major chronic diseases caused by smoking and drinking was serious,which was much higher in men than in women.The disease burden caused by smoking and drinking behavior was mainly the years of life lost,which shows that it has a great impact on the premature death of the population.Controlling smoking and drinking can help to reduce the death level of the population and improve the life expectancy.
