Objective1.To investigate the efficacy and safety of ultrasound-guided percutaneous microwave ablation in the treatment of benign thyroid nodules.2.To investigate the efficacy and safety of ultrasound-guided percutaneous microwave ablation in the treatment of multiple benign breast nodules.Materials and Methods1.A total of 180 nodules were included in 140 patients with benign thyroid tumors who underwent ultrasound guided microwave ablation in our hospital from February 2019 to April 2021.Before treatment,perform routine ultrasound examination,CEUS and US-FNA to clarify the nature of the nodules and design the needle route.group them according to different internal components and diameters,and record the inactivation of the nodules,the volume reduction rate,and the occurrence of complications at 1,3,6,and 12 months after the operation respectively.Analyze the patient’s pain response and aesthetic satisfaction.2.A total of 36 patients with multiple breast nodules diagnosed as BI-RADS 2,3 and 4a by ultrasound were enrolled in our hospital from February 2019 to April 2021.There were202 nodules which confirmed as benign lesions by ultrasound-guided puncture biopsy.All cystic,cystic and solid,solid breast nodules treated at 1,3,6 and 12 months after surgery were recorded for inactivation,safety,and postoperative subjective satisfaction.Analyze the location of the nodule and the level of pain experienced by the patient during treatment.Results1.Follow-up after thyroid nodule ablation 1,3,6,12 months later,VRR were 52.84%,82.11%,92.4%and 97.68%respectively.The average volume before treatment was 5.25(1.87~15.62)cm~3 and 0.13(0.02~0.75)cm~3 at 12 months follow-up.The changes in VRR and volume were statistically significant compared with those before surgery(P<0.05).Cystic nodules are significantly smaller in volume than solid cystic nodules and solid nodules in the first month after surgery.Nodules with a diameter of≤2 cm are significantly smaller in volume than those with a diameter of 2 cm<D≤4 cm and>4cm.The average volume of the three types of nodules at 3,6,and 12 months after surgery has decreased significantly,but there was no significant difference in the volume change of each group.Solid nodules and nodules with a diameter of≤2 cm had the highest percentage of complete ablation in 12 months after surgery.All patients were well tolerated during the treatment,there were 2 cases of skin bruising,2 cases of hoarseness and 1 case of eyelid ptosis occurred in the operative area after the treatment.No serious complications occurred after the treatment.The aesthetic symptom score of postoperative nodules was statistically significant compared with that before operation,and the satisfaction was 100%.2.After 12 months follow-up,the volume of three types of breast nodules was significantly smaller than that before operation(P<0.05).The volume of nodules decreased from(0.55±1.78)cm~3 before operation to(0.03±0.19)cm~3after operation.The VRR at 1,3,6 and 12 months after operation were 62.5%(33.33-100),93.08%(73.33-100),100%(90.4-100)and 100%(100-100)respectively.The total effective rate of ablation was 99.5%(201/202),and the complete ablation rate was 76.2%(154/202).The volume of nodules with different internal structures was reduced at the end of follow-up.The effective rates of ablation for cystic solid nodules and cystic nodules were 100%(62/62,80/80),and the complete ablation rates were 80.6%(50/62) and 80.0%(64/80),The effective rate of solid nodules ablation was 98.33%(59/60),and the complete ablation rate was 66.7%(40/60).There were no serious complications after the operation,and the patients’satisfaction with aesthetics and recovery was 100%.Conclusion1.Ultrasound guided percutaneous microwave ablation treatment technology has the advantages of small trauma,fast recovery,beautiful appearance,easy acceptance by patients,outpatient treatment,and simultaneous ablation of several nodules at one time.2.Ultrasound-guided microwave ablation for the treatment of benign thyroid and breast nodules has significant ablation effects on nodules with different internal structures and different sizes.Cystic nodules and nodules with smaller diameters are more effective.After the ablation,the volume gradually decreased with time,no obvious adverse complications were seen,the treatment was well tolerated,and the subjective satisfaction of the patients was high.On the premise that the patient meets the indications,microwave ablation can be used as an effective method for the treatment of benign thyroid and breast nodules.
