Objective:The study used 1% ropivacaine 3m for local tracheal anesthesia in patients with laparoscopic surgery before intubation,and observed its effect on the stress response during tracheal intubation and extubation in patients with hypertension;explore a prolonged use Anesthesia for hypertensive patients undergoing laparoscopic surgery.Methods:Sixty hypertensive patients(ASA Ⅱ-Ⅲ grade)who underwent laparoscopic surgery in the First Affiliated Hospital of Nanchang University were selected and randomly divided into ropivacaine group(R group)and normal saline group(C group).They were used after induction of conventional anesthesia.The laryngeal tube was intubated with 3ml of ropivacaine or normal saline to the patient under throat and tracheal mucosal surface anesthesia.Observe the patient’s MAP,HR and BIS values at T0 before induction of anesthesia,T1 immediately after intubation,5min T2 after intubation,T3 immediately after extubation,and 5min T4 after extubation,and measure the values of MAP,HR and BIS at each time point at the same time.Cortisol(Cor)concentration(radioimmune method)and blood glucose(Glu)content(glucose oxidase method);record the time from intubation to extubation of the patient,and record the awakening status of the two groups of patients after extubation,and during extubation.The occurrence of coughing at time and after extubation and the patient’s alertness and sedation during the recovery period.Results:The average arterial pressure and heart rate at T1 and T2 of the two groups of patients were significantly lower than those of the patients at T0,and the difference was statistically significant(p <0.05);while the R group was at T3 and T4.The mean arterial pressure and heart rate at point were significantly lower than those in group C,and the incidence of coughing reaction and coughing score in group R were also significantly lower than those in group C,the differences were statistically significant(P<0.05);in addition,group R The breathing recovery time and wake-up time of the patients were earlier than those of group C,with statistical differences(P<0.05).However,there was no statistically significant difference in the sedation scores of the two groups during the recovery period(p>0.05).Conclusion:1% ropivacaine surface anesthesia is applied to hypertensive patients undergoing laparoscopic surgery.Within 6-8 hours after the surface anesthesia of the trachea,it can reduce the patient’s stress response to the extubation operation stimulus,and can better stabilize the height.Intraoperative hemodynamics in patients with blood pressure,and ropivacaine topical anesthesia can significantly reduce the incidence of postoperative coughing,shorten postoperative recovery time,and improve the comfort and safety of patients during the perioperative period.
