| Objective: This study aims to improve the degree of osteoporosis in elderly patients by combining the special features of the third grade a hospital and community general practice. Methods: 1.From June 1,2017 to May 31,2018,the elderly patients who were hospitalized in the community outpatient service of our hospital was screened for osteoporosis,to understand the awareness rate, prevalence rate and treatment rate of osteoporosis, and to analyze and study the risk factors of osteoporosis in the elderly.2.80 elderly patients with osteoporosis were randomly assigned to the control group and the intervention group. The intervention group received the medication guidance, health education, nutrition support, muscle strength and balance ability exercise of the whole professional team at the same time. The control group did not receive systematic intervention.3.After 1 year follow-up,the statistical differences of osteoporosis related indexes between the control group and the intervention group were compared.Results:(1)in some community hospitals in Shanghai, the awareness rate of osteoporosis was 89.7%,the prevalence rate was 39.5%,and the treatment rate was59.5%;(2)multiple factor Logistic regression analysis showed that women, aging, malnutrition, and inability to take care of their daily activities were the risk factors of Op.(3)β-CTX was negatively correlated with BMD,while p1 np,N-MID and 25(OH)Vit D were positively correlated with BMD.(4)One year later, the comparison within and between groups showed that BMD increased, bone metabolism index improved,Barthel scale score increased, nutritional status improved, muscle strength and balance ability improved.Conclusion:(1)the risk factors of OP are female, aging, malnutrition and inability to take care of their daily activities;there is a certain correlation between bone turnover markers and BMD.(2)The osteoporosis degree of the elderly patients was improved through the combination of all specialties. |
