Objective:To explore the influencing factors of postoperative hypoparathyroidism for differentiated thyroid cancer,establish a comprehensive prediction model for the risk of postoperative hypoparathyroidism for differentiated thyroid cancer,and look forward to providing a relatively novel auxiliary reference for the evaluation of differentiated thyroid cancer during surgery.Methods : The clinical and Pathological data of 100 patients with differentiated thyroid carcinoma from September 2018 to December 2019 were retrospectively analyzed,and the patients were divided into hypoparathyroidism group and non-hypoparathyroidism group according to PTH in the peripheral blood.Statistically significant indexes(P<0.05)were included in multivariate Logistic regression analysis.The comprehensive prediction model for hypoparathyroidism after radical operation of differentiated thyroid carcinoma was constructed by multifactor analysis with statistically significant indexes(P<0.05),and the receiver operation characteristic curve was applied to evaluate the Performance of the model.Results : Univariate analysis showed that the type of surgical,preservation of parathyroid glands in situ,parathyroid gland auto-transplantation and lymphatic metastasis were risk factors of hypoparathyroidism.Multivariate analysis showed that the type of surgical,preservation of parathyroid glands in situ and lymphatic metastasis were independent predictors of hypoparathyroidism after radical operation of differentiated thyroid carcinoma.Comprehensive prediction model for hypoparathyroidism after radical operation of differentiated thyroid carcinoma was P=Exp(2.293-2.350X1-1.934X2-1.425X3)/(1+Exp(2.293-2.350X1-1.934X2-1.425X3))(X1= the type of surgical,X2=preservation of parathyroid glands in situ,X3=lymphatic metastasis).The area under the curve,cut-off value,sensitivity and specificity of the predictive model of hypoparathyroidism after radical operation of differentiated thyroid carcinoma were 87.0%,0.344,80.6% and 86.5%respectively.Conclusion : The comprehensive prediction model of postoperative hypoparathyroidism for differentiated thyroid cancer has a good predictive ability for the risk of postoperative hypoparathyroidism,and provides a reference for the evaluation of differentiated thyroid cancer during surgery.
