Objective Through a retrospective study on the medical records of patients with erythema multiform(EM),the clinical characteristics and that of traditional Chinese and Western medicine treatment were summarized and analyzed,so as to accumulate data for further improving the level of diagnosis and treatment of EM in traditional Chinese and Western medicine.Methods To study the characteristics of clinical data of EM and consult medical records,a retrospective study was conducted on 83 cases of EM hospitalized in the Department of Dermatology,Affiliated Hospital of Tianjin Academy of Traditional Chinese Medicine from February 2017 to January 2019.Using statistical methods to obtain relevant results and analysis,and summarize relevant clinical experience.Results 1.Basic data: In this study,83 cases were collected,including 31 males and 52 females,with sex ratio of 1:1.68,the average age was 44.72 ± 17.49,and the average hospitalization days were 10.98 ± 4.09.The total hospitalization time in 2018 was less than that in 2017.2.Clinical features: Among 83 patients,69 cases(83.1%)had the course of disease of 1-10 days,most of which belonged to the acute course;25 cases(30.1%)had infectious factors and 41 cases(49.4%)had drug factors,which showed that the main cause of EM was caused by infection and drug factors.Erythema and papules were the main clinical features,61 cases(73.5%)had target lesions,pruritus was the main discomfort,34(41.0%)had fever or fatigue.3.Laboratory examination: 34 cases(41.0%)of NEUT increased,32(38.6%)of LYMPHN decreased,25(30.1%)of EON decreased,22(26.5%)of ESR increased,33(39.8%)of CRP increased,33(39.8%)of STP decreased,22(26.5%)of SGOT decreased,21(25.3%)of LDH increased,and other changes accounted for less.4.Treatment: glucocorticoid therapy is the main treatment,supplemented by traditional Chinese medicine decoction.Among 83 patients,23 were treated with glucocorticoid alone,49 with glucocorticoid combined with traditional Chinese medicine decoction and 11 with traditional Chinese medicine decoction alone.Statistical analysis was made according to the standard of effective rate after treatment.Glucocorticoid combined with traditional Chinese medicine decoction has a good effect on erythema multiforme with few adverse reactions.By using TCMISS,it is concluded that the traditional Chinese medicine decoction used in our hospital for the treatment of erythema multiform is mainly composed of Chinese medicine for clearing away heat and detoxification,eliminating wind,relieving itching and promoting pharynx,promoting blood circulation,cooling blood and nourishing blood,which is basically the same as the traditional Chinese medicine used in the literature for the treatment of EM.But the multiple usage of traditional Chinese medicine for invigorating blood and Invigorating Qi in the literature is slightly different from the traditional Chinese medicine for clearing heat and detoxifying used in our hospital.The principles of traditional Chinese medicine are basically the same.There are some differences between them and the literature through meridian analysis.Conclusion 1.Erythema multiforme is a kind of acute inflammatory skin disease.The typical lesions are iridoid or target erythema.The inducements are mostly related to upper respiratory tract infection and drug allergy.2.The rapid development of our hospital in the treatment of EM depends on glucocorticoids and traditional Chinese medicine decoction,which is a major feature of our hospital in the treatment of EM.However,there is a lack of evaluation criteria for the efficacy of traditional Chinese medicine in the treatment of EM.According to the evaluation standard of Western medicine curative effect,statistical analysis shows that the combination of Chinese and Western medicine in the treatment of erythema multiforme has good curative effect and less adverse reactions.3.The traditional Chinese medicine used in the treatment of EM in our hospital is almost the same as that used in the literature.The differences can be learned from each other according to the patient’s condition,improving each other. |
