| Objective: To investigate the prevalence of hyperuricemia and the distribution of uric acid levels among rural middle-aged and elderly population in northern China,and to understand the influence factors of hyperuricemia and uric acid levels.The results will provide guidance suggestion for the prevention and treatment of hyperuricemia in rural areas.Methods: A stratified cluster random sampling method was used to select a total of 3135 Tianjin rural residents over 50 years old from the entire cohort of 14583 as the research object.Two subjects without uric acid levels,five with mental illness,and nine with severe kidney disease were excluded.Finally,3119 subjects(1392 men and 1727 women)were included.The research content mainly includes questionnaire surveys,physical examinations and laboratory tests.All participating researchers have undergone unified training,and all participating research subjects have signed written informed consent.This study uses a pre-designed questionnaire to collect general demographic information,lifestyle,and illness information of the objects in a face-toface manner.Physical examination includes general physical examination and image examination(including carotid ultrasound and renal ultrasound).Image examination is performed by a professional imaging physician.Fasting venous blood and venous blood 2 hours after taking sugar were taken for laboratory tests.The diagnosis of metabolic syndrome refers to the diagnostic criteria of the Chinese population.The glomerular filtration rate is calculated from related indicators.SPSS 25.0 was used for statistical analysis.Continuous variables were expressed as mean ± standard deviation(x ± s),and categorical variables were expressed as numbers(frequency).Multivariate analysis was performed for the influence factors of hyperuricemia and uric acid quartile levels by using binary logistic regression and multiple logistic regression,respectively.A two-tailed P value <0.05 was considered statistically significant.Results:1.Prevalence of HUA: The overall prevalence of HUA in the study population was 14.4%,of which the prevalence of men was 14.2%,and the prevalence of women was 14.5%.There was no statistical difference between men and women(χ2 = 0.060,P = 0.806);With the increase of age,the prevalence of HUA also increased(χ2 = 24.661,P <0.001).2.Distribution characteristics of uric acid levels: the overall average level of uric acid is 297.82 ± 85.88 μmol/L,the average level of uric acid in men is higher than that of women;in the quartile group of uric acid levels,the proportion of male uric acid levels is increasing,and women happen on the contrary,the difference was statistically significant(χ2 = 279.988,P <0.001);with the increase of age,the uric acid level showed an increasing trend(χ2 = 44.324,P <0.001),especially in people over 70 years of age.3.Factors Affecting HUA: The results of two-class Logistic regression analysis showed that obesity,metabolic syndrome,hyperlipidemia,waist circumference,2 hours postprandial plasma glucose,white blood cells,and urea nitrogen were independent risk factors for HUA(all P <0.05).Renal Globular filtration rate is an independent protective factor for HUA(P <0.001).4.Factors Affecting Uric Acid Levels: Multiple ordered logistic regression analysis results show that: men,drinking,overweight,obesity,metabolic syndrome,hyperlipidemia,waist circumference,history of cerebrovascular disease,white blood cells,urea nitrogen are independent risk factors affecting uric acid levels(all P <0.05),smoking and glomerular filtration rate were protective factors of uric acid level(all P <0.001).Conclusion: The prevalence of HUA in middle-aged and elderly people in rural areas of Tianjin is high.People with obesity,metabolic syndrome,hyperlipidemia,or with abnormal metabolic indicators(such as WC,2h-PG,WBC,and BUN),those will have a certain early warning effect for being HUA.Those older,obese men who drink alcohol are high-risk groups with elevated uric acid levels.It is necessary to strengthen the health education of this group,urge them to cultivate good habits of life,exercise properly,control waist circumference and weight,which are very important for the prevention and treatment of HUA. |
