| Objective:By comparing unicompartmental knee arthroplasty(UKA)and high tibial osteotomy(HTO)in the treatment of unicompartmental osteoarthritis of the knee,we conducted a clinical investigation to find out the short-term efficacy of the two procedures’s therapeutic effects on the disease.Our results will provide more data to assist surgeon for choosing more suitable surgical methods to achieve optimal effectiveness.Methods:From July 2016 to December 2018,34 patients with single compartment osteoarthritis of the knee were enrolled in the Department of Orthopedics of the first people’s Hospital of Yichang City.No significant differences in age,Sex ratio,body mass index and other basic data of enrolled patients.The patients were randomly divided into two groups and different surgical methods were selected,10 males and 7 females(right knee 8,left knee 9)were grouped into UKA group(mean age 63.1±7.2years,BMI 24.7±2.2 kg/m~2,BMI mean 24 kg/m~2),9 males and 8 females(right knee 10,left knee 7)were grouped into HTO group(mean age 55±79 years,BMI 24.7±2.2 kg/m~2,BMI mean 25kg/m~2).The two groups were treated by single condylar replacement and high tibial osteotomy respectively.All patients were operated on one side.UKA operation was performed in group A and HTO operation was performed in group B,all of them were performed by the same operation and team.To compare and follow up the length of operation,intraoperative bleeding and postoperative drainage volume,the time of beginning ground movement after operation,the days of hospitalization,the angle of knee inverted before and after operation,and the range of knee joint movement before and after operation,3 months and 6 months after operation,and compared and followed up the patients in two groups.Visual analogue score of knee pain,lysholm score of knee joint function,knee joint score of American Special surgery Hospital and American knee Association score.Results:All patients successfully completed the operation,34 patients were followed up successfully,The follow-up time was 3 and 6 months after operation.The operation time of UKA group was(85.6±22.1)min,:(127.1±17.2)min,.There was significant difference between the two groups(p<0.01).The intraoperative bleeding volume was(138.2±29.5)ml,HTO and(189.7±29.1)ml,in UKA group and(189.7±29.1)ml,after operation,there was significant difference between the two groups(p<0.01).There was a significant difference between UKA group(2.12±1.23)days after operation and HTO group(28.42±2.61)days after operation.The length of stay was(15.88±1.11)days in UKA group and(15.53±0.87)days in HTO group.There was no significant difference between the two groups(p>0.05).Comparison of knee inverted angle before and after operation:there was no significant difference between the two groups before and after operation(t=0.083,p>0.05).Intra-group comparison:there was significant improvement in knee inverted angle after operation compared with that before operation,the difference was statistically significant(p<0.05).Comparison between groups:there was no significant difference in ROM,VAS,lysholm score,HSS score and KSS score of knee joint before operation and 3 months and 6 months after operation(p>0.05).Intra-group comparison:the ROM,VAS,lysholm score,HSS score and KSS score of knee joint 3months and 6 months after operation were significantly improved compared with those before operation(p<0.05).Conclusion:The short-term efficacy of UKA and HTO in the treatment of osteoarthritis of single compartment knee joint is similar,and satisfactory clinical results can be obtained.However,the former has the advantages of shorter operation time,less intraoperative and postoperative bleeding,less intraoperative trauma,faster recovery,and earlier lower-ground load-bearing activities.The curative effect is satisfactory,and it is worth popularizing in clinical practice. |
