Objective:The expression levels of vimentin(VIM)and vascular cell adhesion molecule-1(VCAM-1)in serum and urine of patients with systemic lupus erythematosus(SLE)were detected to analyze the correlation between VIM and VCAM-1 and patients with SLE and patients with complicated lupus nephritis(LN),and to further explore the relationship between VIM and VCAM-1 and LN activity.Methods:In this study,59 SLE patients and 36 healthy controls were enrolled and the patients with SLE divided into the LN group(45 cases)and the non-LN group(14 cases).The LN group was defined as Group A,the non-LN group as Group B,and the healthy control group as Group C.The expression levels of VIM and VCAM-1 in serum and urine of participants were determined by enzyme-linked immunosorbent assay(ELISA),and the two indexes were compared among group A,B and C,respectively.According to renal SLEDAI score(r SLEDAI),LN patients were divided into active stage LN group(33 cases)and stable stage LN group(12 cases),which were defined as group A1 and group A2,respectively.The expression levels of VIM and VCAM-1 in serum and urine were compared,and the differences of expression levels between the two groups and the correlation with relevant clinical indicators were analyzed.Receiver operating characteristic curve(ROC)was used to evaluate the diagnostic accuracy of VIM and VCAM-1 expression levels for LN and LN activity.Results:1.The serum VIM expression levels were different between groups A,B,and C(P<0.001).The expression level of group A was significantly higher than that of group B(P=0.003)and group C(P<0.001),and group B was significantly higher than group C.(P=0.044);There was no significant difference in the expression level of urine VIM among the three groups(P=0.247).Serum VIM expression was increased in patients with SLE and SLE combined with renal involvement,while urine VIM expression had no significant change.2.The expression levels of VCAM-1 in serum and urine were different between groups A,B,and C(P<0.001,P<0.001),and the levels of VCAM-1 in serum and urine in group A were significantly higher than those in group C(P<0.001,P=0.474),the serum and urine levels of VCAM-1 in group B were higher than those in group C(P=0.019,P<0.001),the expression of serum VCAM-1 between group A and group B There was no significant difference in levels(P=0.474),and the level of urine VCAM-1 in group A was significantly higher than that in group B(P=0.001).The expression level of VCAM-1 in serum and urine of SLE patients was higher than that of healthy control group.The expression level of VCAM-1 in urine was increased in patients with LN,and there was no significant change in serum VCAM-1 compared with patients with SLE without LN.3.The expression levels of urine VIM and serum VCAM-1 were not significantly different between group A1 and group A2(P=0.171,P=0.982).The levels of serum VIM and urine VCAM-1 in group A1 were significantly higher than those in group A2(P=0.038,P<0.001).The serum VIM expression level of LN patients was positively correlated with SLEDAI score and r SLEDAI score(r_s=0.312,P=0.037;r_s=0.311,P=0.038),urine VCAM-1 expression was also significantly positively correlated with SLEDAI score and r SLEDAI score(r_s=0.531,P<0.001;r_s=0.472,P=0.001);4.ROC curves of the expression levels of serum VIM and urine VCAM-1 in the LN group were constructed.The results showed that the areas under the curves of the two indicators were 0.715 and 0.909,and the cut-off values were 17.12ng/m L(sensitivity 48.48%,specificity 91.67%)and 844.21ng/m L(sensitivity 84.85%,specificity 83.33%),respectively,the area under the curve for the combined detection of serum VIM and urine VCAM-1 is 0.922,the sensitivity is and the specificities were 81.82%and 91.67%,respectively.Conclusions:1.The expression levels of serum VIM and urine VCAM-1 were increased in patients with SLE and LN.2.Detection of serum VIM and urine VCAM-1 expression levels in patients with LN has potential clinical predictive value in evaluating the activity of LN.
