Objective: To observe the clinical efficacy of Xiaoke Sanyu Decoction combined with 577 nm laser in the treatment of diabetic retinopathy(DR)with deficiency of Qi and Yin and collagonal blood stasis.By comparing the clinical efficacy of patients in the two groups before treatment and one month after treatment,the effective treatment of DR was further explored,and the integrated treatment of traditional Chinese and western medicine was sought to improve the overall efficacy of DR.Methods: A total of 36 eyes of DR patients with deficiency of Qi and Yin and collagonal blood stasis in the ophthalmology consultation and hospitalization in Guangxi University of Chinese Medicine Affiliated Hospital were selected and randomly divided into the control group and the experimental group,18 eyes in each group.The control group was treated with 577 nm laser photocoagulation,and the experimental group was treated with Xiaoke Sanyu Decoction on the basis of the control group.The best corrected visual acuity(BCVA)was recorded before treatment and 1 month after treatment,respectively.Central macular thickness(CMT)is measured by optical coherence tomography(OCT).Retinal neovascularization,retinal fluorescein leakage and occlusion were examined by fundus fluorescein angiography(FFA),retinal bleeding absorption was examined by fundus photography,and TCM syndrome scores of the two groups of patients were recorded.Results: 1.Before treatment,there were no statistically significant differences in age,gender,DR stage,BCVA,CMT and TCM syndrome scores between the two groups(P=0.837,0.738,0.796,0.398,0.669,0.797).The age of the control group was(61.39±8.89)years old.BCVA(0.33±0.18);CMT(329.44±105.82)microns;TCM syndrome score was(27.11±2.56).The experimental group was(60.72±10.36)years old;BCVA(0.38±0.16);CMT(345.22±113.31)microns;TCM syndrome score was(26.89±2.59).2.Comparison of BCVA after treatment: BCVA in the control group(0.45±0.23)after treatment was higher than that before treatment(0.33±0.18)(Z=-3.106,P=0.002,n=18);BCVA in the experimental group(0.58±0.15)after treatment was higher than that before treatment(0.38±0.16)(Z=-3.764,P=0.000,n=18).BCVA in the experimental group was better than that in the control group after treatment(Z=-1.994,P=0.046).3.Comparison of CMT after treatment: the CMT of the control group after treatment was improved(237.56±53.76)μm compared with that before treatment(329.44±105.82)μm(Z=-3.724,P=0.000,n=18);The CMT of the experimental group was improved(211.17±35.23)μm after treatment compared with that before treatment(345.22±113.31)μm(Z=-3.636,P=0.000,n=18).The CMT of the experimental group after treatment was better than that of the control group(Z=-2.010,P=0.044).4.Comparison of FFA after treatment: the effective rate of the experimental group was 83.3%,and that of the control group was 72.2%;The retinal neovascularization,retinal fluorescence leakage and occlusion in the experimental group were better than those in the control group(Z=-2.018,P=0.04).5.Comparison of the absorption of retinal fundus hemorrhage after treatment: the effective rate was 94.4% in the experimental group and 88.9% in the control group;The absorption of retinal fundus hemorrhage in the experimental group was better than that in the control group(Z=-2.201,P=0.028).6.Comparison of TCM syndrome scores after treatment: The TCM syndrome scores of the control group after treatment(15.28±1.96)were improved compared with those before treatment(27.11±2.56)(Z=-3.734,P=0.000,n=18);The TCM syndrome score of the experimental group after treatment(11.83±2.64)was improved compared with that before treatment(26.89±2.59)(Z=-3.733,P=0.000,n=18).Among them,the TCM syndrome score of the experimental group was better than that of the control group after treatment(Z=-3.535,P=0.000).7.Comparison of efficacy of traditional Chinese medicine: the effective rate of the experimental group was 88.9%,and the effective rate of the control group was 77.8%.Both groups of DR patients were effective after treatment.The therapeutic effect of TCM in the experimental group was better than that in the control group(Z=-2.147,P=0.032).Conclusion: Compared with Xiaoke Sangyu Decoction combined with 577 nm laser in the treatment of DR,the combination of traditional Chinese medicine and laser can effectively improve the patient’s vision,reduce retinal edema and promote the absorption of retinal hemorrhage.It was concluded that Xiaoke Sanyu Decoction can increase the therapeutic effect and shorten the hospitalization time of patients on the basis of 577 nm laser treatment of Qi and Yin deficiency and collateral-vein stasis syndrome DR. |
