Impact Of Balint Group On Communication Ability Of Nurses In Discipline Training

Objective: To understand the current situation of the communication skills of nurses trained in a tertiary hospital in a province.To explore effective ways to improve the communication skills of nurses.To explore the different effects of Balint group and traditional training on improving the communication skills of nurses.Methods: Applying nurses’ clinical communication ability scale to investigate the clinical communication ability of regular training nurses in a part of third-grade hospitals in a province.120 paper questionnaires were issued and recovered,of which114 were valid questionnaires and 326 were received online,all of which were valid questionnaires.The total number of valid questionnaires is 440.According to the principle of voluntary registration,30 nurses with lower scores were selected from the nurses with clinical communication ability of less than 4 points in a hospital using a random number table method,and they were assigned to the study group,control group and blank control group.There are 10 people in each group.The study group and control group received Balint group activities and general nursing communication education for8 consecutive weeks.The blank group did not intervene for the time being.Nurses’ clinical communication ability scale,Jefferson empathy scale(HP version)and self-efficacy scale were used before and after the intervention to compare the differences between the three groups.Results: 1.The survey results showed that the total average score of clinical communication ability of regular training nurses nurses was 3.97±0.36,the score belongs to above average level,but lower than that of general nurses(4.21±0.83)(t=-6.184,P=0.000).2.There were significant differences among the regular training nurses of different genders in the total mean score of clinical communication ability(P=0.019),basic verbal communication ability(P=0.031),basic non-verbal communication ability(P=0.029)and difficult situation communication ability(P=0.023).Male nurses(4.06±0.43)were significantly higher than female nurses(3.95±0.34)in the total mean score of communication ability.Male nurses(4.12±0.46)were significantly higher than female nurses(3.99±0.45)in basic language communication ability.The basic non-verbal communication ability of male nurses(4.12±0.51)was significantly higher than that of female nurses(4.00±0.37).The communication ability of male nurses(3.99±0.48)was significantly higher than that of female nurses(3.87±0.38).3.There were significant differences in clinical communication ability(P=0.049),basic non-verbal communication ability(P=0.026)and team communication ability(P=0.002)among the regular training nurses with different marital status.Specifically,married nurses(4.13±0.36)had a significantly higher mean score than unmarried nurses(3.96±0.35).The basic nonverbal communication ability of married nurses(4.24±0.43)was significantly higher than that of unmarried nurses(4.02±0.39).Married nurses(4.24±0.43)showed significantly higher team communication ability than unmarried nurses(4.06±0.39).4.There were significant differences in team communication ability among regular training nurses in different departments(P=0.004).The team communication ability of nurses in ICU and operating room(4.23±0.45)was significantly higher than that of nurses in internal medicine(4.07±0.39),surgery(4.02±0.38),obstetrics,gynecology and pediatrics(4.11±0.40),emergency department(3.92±0.30)and other departments(4.10±0.39).5.There was no significant difference in clinical communication ability among regular training nurses with different age,household registration,educational background and standardized training time(P=0.74,0.982,0.840,0.305).6.Before intervention: The total mean scores of clinical communication scale in the study group,control group and blank control group were 3.58±0.20,3.48±0.26 and 3.43±0.37,respectively,and the difference was not statistically significant;The total mean scores of the empathy scale in the study group,the control group and the blank control group were 5.44±0.53,5.37±1.02 and 5.16±0.68,respectively,and the difference was not statistically significant;The self-efficacy scale scores of the study group,control group and blank control group were 2.38±0.27,2.20±0.24 and 2.33±0.30,respectively,and the difference was not statistically significant.7.After intervention: Univariate analysis of variance showed that there were significant differences between the study group,the control group and the blank control group in the communication ability scale(3.94±0.19,3.54±0.19,3.57±0.24,P=0.000)and the empathy scale(5.78±0.44,5.20±0.63,4.87±0.80,P=0.013),and the differences were statistically significant.LSD test was used to analyze and compare pairwise within the groups.The results showed that there were significant differences in communication ability and empathy scales between the study group and the control group(P < 0.05),except that there were no significant differences in basic non-verbal communication ability,total mean score of empathy and opinion selection(P > 0.05).There were significant differences between the study group and the blank control group in all dimensions(P < 0.05).There were significant differences between the control group and the blank control group in the dimension of viewpoint selection(P < 0.05).In the scale of self-efficacy,there was no significant difference between the three groups(P>0.05).8.Before and after intervention,there were significant differences in clinical communication scale(P=0.002)and empathy scale(P=0.037)in the study group,and the differences were statistically significant.In the control group,there were significant differences in communication ability in difficult situations(P=0.045)before and after intervention,and the difference was statistically significant.There was no significant difference in the scores of each scale in blank control group(P > 0.05)before and after8 weeks,and the difference was not statistically significant.9.The comparison of the difference in scale scores between the study group and the control group before and after the intervention shows that the study group and the control group have the total average score of communication ability(P<0.05),basic language communication ability(P<0.01)and emotional perception ability(P<0.01)There are significant differences in the differences of the three dimensions before and after intervention,and the differences are statistically significant.Conclusion:1.Compared with the general nurses,the regular training nurses have a lower level of clinical communication ability.2.Balint group can effectively improve the clinical communication ability and the level of empathy of regulatory nurses,but the effect on the improvement of self-efficacy needs further study.3.Compared with the general communication ability education and training of nurses,the intervention effect of Balint group is better.
