Observation On The Curative Effect Of Qiangji Tongdu Decoction In The Treatment Of Residual Low Back Pain After OVCF Percutaneous Vertebroplasty

Objective:To observe the clinical efficacy of Qiangji Tongdu Decoction in the treatment of residual low back pain after OVCF percutaneous vertebroplasty through prospective randomized controlled trial,and to analyze and discuss the mechanism of Qiangji Tongdu Decoction in the treatment of this disease,so as to provide a therapeutic method for clinicians in the treatment of residual low back pain after OVCF vertebroplasty.Methods:Patients diagnosed with OVCF in Suzhou Hospital of Traditional Chinese Medicine from September 2018 to September 2020 who had residual low back pain after PVP were selected.According to inclusion and exclusion of standard vertebroplasty,60 patients who met the criteria were divided into the traditional Chinese medicine treatment group,celecoxib treatment group and Gushukang treatment group,with 20 cases each.All three groups were treated with conventional anti-osteoporosis basic drugs,and the traditional Chinese medicine group was treated with Qiangji Tongdu Decoction plus or minus prescription.The celecoxib treatment group was given oral celecoxib capsules.Gushukang treatment group was given Gushukang granules orally 14 days was used as a course of treatment.After six courses of treatment,drug withdrawal was followed up to 6 months after initial treatment.All patients were followed up at the time of initial diagnosis,2 weeks,4 weeks,3 months and 6 months after treatment,and the efficacy indexes were recorded.Statistical Methods SPSS 21.0 software was used for data statistics,analysis and processing.Results:After excluding factors such as age,gender and surgical segment,statistical analysis was conducted on the data at each time point before and after treatment in the Chinese medicine group,celecoxib group and Gushukang group,and the results showed that:①In terms of pain improvement,the VAS score of the Chinese medicine group at 6 months after treatment was 2.65 ± 0.71,that of the celecoxib group at 6 months after treatment was 1.95±0.65,and that of the Gusukang group at 6 months after treatment was 2.85±0.75.All the three groups had therapeutic effects(P<0.05),but the comparison between the three groups showed that,The therapeutic effect of celecoxib group was better than that of traditional Chinese medicine group and Gushukang group(P<0.05),and the therapeutic effect of traditional Chinese medicine group and Gushukang group was similar(P>0.05).②In terms of improving the function of lumbar and back muscles,the ODI score of the Chinese medicine group at 6 months after treatment was 23.70±5.25,the ODI score of the celecoxib group at 6 months after treatment was 28.00±5.43,and the ODI score of the Gushukang group at 6 months after treatment was 26.30±5.47.All the three groups had therapeutic effects(P<0.05).The therapeutic effect of the Chinese medicine group was better than the other two groups,and the therapeutic effect of the Gushukang group was better than that of the celecoxib group.③Activities of daily living were not improved in the three groups,and there was no statistical significance among all groups(P>0.05).④In terms of improving bone mineral density,the bone mineral density of the Chinese medicine group at 6 months after treatment was-2.56±0.46,and that of the Gushukang group at 6 months after treatment was-2.65±0.45,both groups had therapeutic effect(P<0.05),and there was no significant difference between the Chinese medicine group and the Gushukang group(P>0.05).The results indicated that the traditional Chinese medicine group and Gushukang group had similar effect on improving bone mineral density.BMD of the celecoxib group at 6 months after treatment was-2.92±0.45,compared with-2.80±0.33 before treatment,indicating that the celecoxib group was not effective in improving BMD.⑤There was no significant effect in improving the anterior edge height and Cobb Angle of the injured vertebrae in the three groups(P>0.05).⑥The scores of TCM symptoms were 4.95±1.53 in the Chinese medicine group,6.25±1.33 in the celecoxib group and 5.95±1.47 in the Gushukang group at 6 months after treatment,with statistical significance among the three groups(P<0.05).Compared with Gushukang group and celecoxib group,there was statistical significance in Chinese medicine group(P<0.05).⑦ After treatment,the total effective rate of the Chinese medicine group was 90%,the total effective rate of the celecoxib group was 80%,and the total effective rate of the Gushukang group was 90%,and there was no significant difference between the two groups(P>0.05).Conclusion:Qiangji Tongdu decoction can effectively treat the residual low back pain after OVCF percutaneous vertebroplasty.It has significant effect on pain relief and recovery of low back function,and has the effect of anti osteoporosis.It can promote the recovery of disease and further improve the quality of life of patients.It is worthy of clinical application.
