Objective:To observe the effects of different doses of oxycodone on patient-controlled intravenous analgesia(PCIA)after gynecological laparoscopy.Methods:60 patients with elective laparoscopic gynaecological surgery were randomly selected,ASA I-II.Using the random number table method,the patient were divided into groups S,O1,O2 and O3,15 patients in each group.In group S,sufentanil was administered intravenously when the abdomen was closed.0.1mg/kg oxycodone was injected intravenously in the O1-O3 group at abdominal occlusion.The postoperative PCIA pump was started after demining(drug preparation: 2ug/kg dexmedetomidine and 12.5mg dolasetron were combined with 1 g/kg sufentanil and0.5mg/kg,0.75mg/kg/kg oxycodone.The mixture was produced with 0.9%normal saline in 100 ml,the background infusion rate was 2ml/h,the single press dose was 1ml and the locking time was 15 min.VAS scores at2 h,4h,12 h,24h and 48 h after the operation were observed and compared.Effective PCIA compression times,analgesia satisfaction(satisfactory,relatively satisfactory,less satisfactory,dissatisfied),skin itching,nausea,vomiting,respiratory depression,dizziness and other side effects were recorded within 48 hours of surgery.Results:There were no significant differences in age,weight,height,ASA classification,operating time,anesthesia time and total fluid intake in 4groups(P>0.05).VAS values in groups O2 and O3 were significantly lower at all times than in group O1(P<0.05)and there was no statistical significance between O2 and O3 groups(P<0.05).The values in group S were significantly higher at T5 than in groups O2 and O3 with statistical significance(P<0.05).The effective times of pressing within 48 hours in the O1 group were(3.67 ± 1.81)times,(0.67 ± 1.01)times in the O2 group,(1.53±1.75)times in the O3 group and(3.73±1.44)times in the S group.The O1 group was significantly higher than the O2 and O3 groups with statistical differences(P<0.05);the S group was significantly higher than the O2 and O3 groups with statistical differences(P<0.05);there was no statistical difference between group O1 and S,and there was no statistical difference between group O2 and group O3(P>0.05).The incidence of dizziness in groups O3 and S was significantly higher than in groups O1 and O2,and the difference was statistically significant(P<0.05).There was no statistical significance between groups O3 and S and no statistical significance between groups O1 and O2(P>0.05).There was no statistical significance for the incidence of skin itching with nausea and vomiting in the four groups(P>0.05).There was no significant difference in satisfaction with analgesia(P >0 0.05)in the four groups.Conclusions:0.75mg/kg oxycodone has good analgesic effect and low incidence of adverse reactions in PCIA during gynecological laparoscopic surgery. |
