ObjectiveTo observe and compare the effects of sevoflurane and desflurane on emergence agitation(EA)in elderly patients undergoing traumatic fractures of extremities during recovery period.methods1.120 patients undergoing orthopedic surgery at the Hospitalfrom January2020 to May 2020.2.Collect the case data of each patient,including gender,age,height,weight,BMI index,ASA,graded surgery types,MMSEand other general materials.The patients who met the criteria for inclusion were divided into sevoflurane group(Sevoflurane,group S)and desflurane group(Desflurane,group D)according to the principle of randomization,with 60 cases in each group.3.According to the Ricker agitation-sedation scale(RSS),the patient’s agitation immediately,1h,and 2h after the operation was evaluated;according to the Nmeric rating scales(NRS),the immediate,1h,2h after the operation was recorded Pain degree;record the value of Partial Pressure of Carbon Dioxide(PaCO2)at the beginning of the operation,immediately after the operation,1h,2h;observe and analyze the operation time,anesthesia time,recovery time,extubation time and blood loss;Post-operative adverse reactions such as chills,nausea and vomiting(PONV),etc.were also followed up.4.Use SPSS23.0 statistical software to analyze and compare the effects of sevoflurane and desflurane on the agitation of elderly patients undergoing limb traumatic fracture surgery.Results1.General information: There were no statistically significant differences in gender,body mass index(BMI),ASA grade composition,operation type composition,and preoperative MMSE score between the two groups of patients(P>0.05).2.Pain degree: The two groups of patients were compared at different time points immediately after surgery,1h after surgery,and 2h after surgery.There was no statistically significant difference in pain scores(P>0.05).3.Degree of agitation: Comparison of agitation scores at different time points after operation between the two groups.In comparison within the group,the agitation scores at 1h and 2h after the operation were lower than that immediately after the operation(P<0.05).In comparison between the groups,the agitation scores of group S were significantly lower than those of group D immediately,1h,and 2h after operation(P<0.05).4.Blood gas level: There was no statistically significant PaCO2 difference between the two groups of patients at the beginning of the operation,immediately after the operation,1h after the operation,and 2h after the operation.5.General operation conditions: There was no statistically significant difference in surgical blood loss,operation time,anesthesia time,and extubation time between the two groups(P>0.05).In terms of wake-up time,group S was significantly longer than group D(P<0.05).6.Postoperative adverse reactions: The incidence of chills,PONV,BP increase,and HR increase in the two groups was not statistically significant(P>0.05).There were no serious adverse reactions such as forced body movement,incision dehiscence,increased bleeding,and self-extraction of catheters in both groups.Conclusions1.Using desflurane to anaesthetize elderly patients undergoing general anesthesia on limbs,postoperative recovery is faster than using sevoflurane,which improves the quality and safety of recovery.2.Using desflurane to anesthetize elderly patients undergoing general anesthesia on limbs,the degree of agitation in the postoperative recovery period is slightly heavier than that of sevoflurane,and more attention should be paid and preventive and therapeutic measures should be taken.3.Using desflurane and sevoflurane to anesthetize elderly patients undergoing general anesthesia of extremities,the incidence of postoperative complications is basically the same.prevent the dangerous complications.
