| Background and purpose: The core of hospital management is quality,and medical quality is directly related to the health rights of the people.Continuous improvement of quality and protection of medical safety are an important content and foundation for the reform and development of health services.In recent years,medical accidents have occurred frequently,doctor-patient relations have been strained,and hospital credibility has declined.Therefore,continuously standardizing hospital quality management,improving hospital quality,and meeting patient needs have become hot issues that urgently need to be resolved.Therefore,the establishment of a medical quality evaluation index system for Chinese Medicine Hospital and the overall management of Chinese Medicine Hospitals’ medical quality can improve the management level of Chinese Medicine Hospital and the quality of medical services,and harmonize the relationship between doctors and patients.Therefore,this study collects data to improve the medical quality evaluation indicators of Chinese Medicine Hospital,and uses the Donabedian model to build medical quality evaluation index system of "structure-process-results" of Chinese Medicine Hospital,so that the existence of indicators is not only for health statistics,but more focused on medical care.The quality itself.Objectively reflect the level of hospital medical quality management work,play an evaluation and guiding role,improve the extensive medical quality management of Chinese Medicine Hospital,achieve an objective,fair and accurate evaluation of medical quality of Chinese Medicine Hospital,improve medical quality and service levels,and meet patient needs.Methods: This study uses the literature research method to collect domestic and foreign related literature on medical quality evaluation,and searches the official websites of the Ministry of Health to collect medical quality evaluation indicators.Through literature analysis and panel discussion,the indicators are screened and the initial framework of the indicator system is constructed.Questionnaire surveys are conducted through the expert consultation method,and the rationality of the framework is analyzed by using statistical principles and methods to finally construct a "structure-process-result" medical quality evaluation index system for of Chinese Medicine Hospital.Use the relevant data of 5 departments of A hospital to carry out case analysis,compare the score of the comprehensive evaluation with the score of the actual medical condition of the hospital,and verify the scientificity,feasibility and applicability of the index system.Results: The effective recovery rate of the expert consultation questionnaire was 100%,and the expert positive coefficient was100%;the expert authority coefficient(Cr)of the first-level index was 0.800,0.763,and 0.756,and the expert authority coefficient(Cr)of the second-level index was 0.788,0.750,0.769,0.769,0.775,0.750,0.781,0.769,0.744,0.756.Experts are highly authoritative.The coordination coefficients of expert opinions of the first,second and third levels were 0.276,0.262,and 0.144 respectively,all of which were statistically significant(P<0.05),and the coordination and consistency of expert opinions were good.Keep the index with a coefficient of variation of less than 0.25.After the panel discussion,some indicators were deleted.The final medical quality evaluation index system framework for of Chinese Medicine Hospital includes 3 first-level indicators,9second-level indicators,and 30 third-level indicators,including basic quality,link quality,and terminal quality.This study conducted case analysis by collecting data,using the ICC intra-group correlation coefficient method to study the consistency of the scores based on the comprehensive evaluation method and the actual medical conditions of the hospital.The results showed that the ICC correlation coefficient was 0.453(95%CI: 0.059-0.799),which was higher than 0.4,indicating that the consistency of the evaluation results was moderate.This shows that the results of the comprehensive evaluation are basically consistent with the actual medical quality of the hospital,and the indicator system can reflect the actual medical quality.Conclusion: The medical quality evaluation index system of Chinese Medicine Hospital constructed in this study includes 3 first-level indicators,9 second-level indicators,and 30 third-level indicators: basic quality,link quality,and terminal quality.The expert consultation results show that experts are highly motivated and authoritative,and the expert consultation coordination coefficient is high,which passed the test.Case research results show that the results of the comprehensive evaluation are basically consistent with the actual medical quality of the hospital,indicating that the indicator system can reflect the actual medical quality and has certain scientificity,feasibility and applicability. |
