Clinical Study On The Visfatin And Nutrition Station Of Stable COPD With Deficiency Of Lung And Spleen By The Method Of Nourishing Lung And Strengthening Spleen

Objective:To observe the effect with the method of nourishing lung and strengthening spleen(Yupingfeng powder and Liujunzi decoction)on visfatin level,nutritional status,lung function,inflammatory indicators,CAT score and TCM syndrome score in patients of stable COPD with deficiency of lung and spleen,and to provide new ideas and methods for clinical evaluation and treatment of COPD.Methods:A total of 76 patients with stable COPD who met the standards were divi ded into the experimental group(supplemented with yupingfeng sanhe liujunzi decoction)and the control group(supplemented with yupingfeng sanhe liujunzi decoction)by random number table method.Every group included 38 patients.The changes of visfatin level,nutritional status,lung function,inflammatory indicators,CAT score and TCM syndrome score were compared before and after treatment.SPSS 23.0 software was used for statistical analysis of the obtained data.Results:After treatment,BMI,MAC,TP and ALB in the experimental group were increased compared with those before treatment,and the difference was statistically significant(P<0.01);TP in the control group was increased compared with that before treatment,and the difference was statistically significant(P<0.05);BMI,MAC and ALB were not significantly improved compared with those before treatment,and the difference was not statistically significant(P>0.05).The levels of visfatin in the experimental group were increased compared with those before treatment,and the difference was statistically significant(P<0.01).The levels of visfatin in the control group were not significantly changed compared with those before treatment,and the difference was not statistically significant(P>0.05).There was no significant difference in WBC and NEUT%between the experimental group and the control group(P>0.05).FEV1,FEV1/FVC and IC in the experimental group and the control group were increased compared with those before treatment,and the difference was statistically significant(P<0.01).In terms of improvement degree,FEV1 in the experimental group was higher than that in the control group,and the difference was statistically significant(P<0.01).The total effective rate of TCM syndrome was significantly higher in the experimental group than in the control group(P<0.01).The TCM syndrome score and CAT score of the two groups were improved after treatment compared with those before treatment,and the difference was statistically significant(P<0.01).The improvement degree of the experimental group was more significant than that of the control group(P<0.01).Before treatment,visfatin in all COPD patients were positively correlated with BMI,TP,ALB,WBC,FEV1,FEV1/FVC and IC,and negatively correlated with disease course.BMI was positively correlated with visfatin,MAC,TP,ALB,WBC,and FEV1/FVC,and negatively correlated with disease course.After treatment,visfatin in all COPD patients were positively correlated with BMI,MAC,TP,ALB,WBC,FEV1,IC,and FEV1/FVC.BMI was positively correlated with visfatin,MAC,TP,ALB,FEV1,FEV1/FVC and IC,and negatively correlated with disease course.Conclusion:1.The method of nourishing lung and strengthening spleen(yupinfeng sanhe liujunzi decoction)can more effectively improve the clinical symptoms and nutrition station of stable COPD with deficiency of lung and spleen,improve the quality of life,and no obvious adverse reactions were observed in the treatment process.2.visfatin may take part in the development of nutrition station in COPD.
