Objective:To observe the clinical characteristics of ulcerative colitis(UC)and to investigate the patient’s adherence to treatment(A2T)and quality of life(QOL)in UC patients..Methods:The inpatients with UC in the First Affliated Hospital of Fujian Medical University from January 2014 to December 2019 were included, and the extent and severity of disease, extra intestinal manifestations(EIMs),complications, medical managements, laboratory indexes, endoscopic and histopathological findings were recorded respectively.Morisky Scale and IBDQ questionnaires were respectively used to evaluate the patient’s adherence to treatment and the QOL in UC patients.Results:1.A total of 261 UC patients were enrolled, including 160 males and 101females.The age of onset was 43(31,52)years old.The inflammation extent was positively correlated with the severity of symptoms,(r=0.493,P<0.001),such as abdominal pain(r=0.261,P<0.001),fever(r=0.311,P<0.001),anemia(P=0.002).Patients with extensive colitis were more likely to show weight loss(P=0.002),fatigue(P=0.029),and lower gastrointestinal bleeding(P=0.005).There was correlation between clinical and endoscopic activity(r clinical VS endoscopic=0.289,P<0.001),and between endoscopic and histological activity(r clinical VS endoscopic=0.177,P=0.007)respectively.ESR and PLT count showed some certain predictive value on the disease activity and endoscopic activity.CRP level showed different in patients with different disease activities, and in patients with different endoscopic Mayo scores(P<0.05).2.60.92%of patients had poor A2T,and poor A2T was associated with disease recurrence(P=0.034);The median IBDQ score was 181(146,195).The IBDQ scores were significantly reduced in female patients(P=0.001),those with lower education level(P=0.049)and with duration of disease≥10 years(P<0.001).Conclusions:In UC patients, clinical symptoms are positively related to the extent of disease. However, clinical symptom relief does not suggest mucosal healing and disappearance of histopathological inflammation. These patients have lower quality of life, and show poor adherence to treatment, which is one of important factors for disease recurrence. Therefore,in clinical practice, more attention should be paid to mucosal healing and histological inflammation regression, compliance and quality of life in addition to symptom relief.
