Objective:Due to the lack of drugs with ideal efficacy for the treatment of non-alcoholic fatty liver disease(NAFLD),current treatment measures are mainly to correct potential risk factors,delay the occurrence and development of NAFLD,look for indicators indicating NAFLD,and improve the early diagnosis of NAFLD.Therefore,this study analyzed the correlation between the level of homocysteine(Hcy)and other relevant factors in patients with T2DM complicated with NAFLD in yanbian area,and provided theoretical basis for the diagnosis,prevention and treatment of T2DM complicated with NAFLD.Research objects and methods:A total of 284 patients with type 2 diabetes who were hospitalized in the Department of Endocrinology,Affiliated Hospital of Yanbian University from April 2018 to December 2019 were selected.The subjects were divided into the DM+NAFLD group and the DM group according to the determination of fatty liver by abdominal ultrasound.The DM+NAFLD group was divided into simple fatty liver group and steatohepatitis group according to whether abnormal liver function was detected by blood sampling.At the same time,138 healthy physical examiners were selected as NC group of healthy control group.Age,gender and ethnicity were recorded to improve clinical indicators,and plasma Hcy,HbAlc,HDL-C,LDL-C,TC,triglyceride(TG)of patients in each group were measured.SPSS26.0 statistical software was used for analysis.Results:1.A comparison of the general influencing factors among the three groups showed that the proportion of males in the DM group and the DM+NAFLD group was higher than that in the NC group,and the difference was statistically significant(P<0.05).Age difference between NC group,DM group and DM+NAFLD group was not statistically significant.(P>,0.05);The proportion of han in the DM+NAFLD group was higher than that in the NC group,and the difference was statistically significant(P<0.05).Comparison of other clinical indicators between the three groups showed that HDL of the DM group and the DM+NAFLD group was lower than that of the NC group,and the difference was statistically significant(P<0.05).Both BMI and LDL in the DM+NAFLD group were higher than those in the NC group and the DM group alone,with statistically significant differences(P<0.05).TG,Hcy and HbAlc levels in the DM+NAFLD group were higher than those in the NC group and the DM group alone,and TG,Hcy and HbAlc levels in the DM+NAFLD group were higher than those in the NC group,with statistically significant differences(P<0.05).There were no statistically significant differences in TG,HDL,LDL and Hcy between Korean and han groups in the DM+NAFLD group(P<0.05).2.Regression analysis results:BMI,TC,TG,LDL,Hcy and age were influential factors for T2DM with NAFLD.Correlation analysis results:Hcy was positively correlated with age,BMI,TG and HbAlc,and negatively correlated with HDL.3.BMI of the simple fatty liver group was lower than that of the steatohepatitis group,and the difference was statistically significant(P<0.05).Conclusion:1.Hcy,TC,TG,LDL,BMI,gender,age and ethnicity were associated with the incidence of T2DM with NAFLD.2.Hcy,TG,LDL,BMI and age were risk factors for T2DM with NAFLD.3.Hcy was positively correlated with age,BMI,TG and HbAlc,and negatively correlated with HDL.The synergistic effect of Hcy with age,BMI,TG and HbAlc may cause the occurrence of NAFLD in T2DM.HDL may be a factor to prevent the formation of T2DM with NAFLD.4.Patients with T2DM complicated with different degrees of NAFLD,and the severity of NAFLD increased with the increase of BMI.
