A Multi-center Clinical Study On The Treatment Of Parkinson's Disease Based On Syndrome Differentiation And Classification Of Traditional Chinese Medicine

Objectives This study aims to observe the effect of traditional Chinese medicine(TCM)in the treatment of various motor symptoms and non-motor symptoms of Parkinson’s disease,and to further observe whether TCM has different therapeutic focus for Parkinson’s disease patients with different syndromes.Methods A randomized,double-blind,placebo-controlled,parallel design was used in this study.292 patients with Parkinson’s disease were divided into control group and treatment group according to the random number table method.In the control group,146 patients were treated with western medicine and 1/20 concentration of placebo.In the treatment group,146 patients were treated with pingzhan fang on the basis of western medicine.At the same time,according to the patients’ symptoms combined with the syndrome differentiation of tongue and vein,the patients were divided into four syndromes,namely,liver and kidney deficiency,qi and blood deficiency,qi stagnation and blood stasis,and phlegm and heat and wind,and corresponding drugs were distributed for different syndromes.Treatment group and control group in the treatment of 8,16,24 weeks of follow-up,compared two groups of patients before and after treatment UPDRS scale score,modified Hoehn-Yahr,elderly quiver of TCM syndrome classification dysfunction score,Hamilton anxiety scale(HAMA)and Hamilton depression scale(HAMD)and so on a scale score,statistics curative effect before and after medication,general situation and adverse reactions occur.Epi Data3.1 software was used to establish a database platform for data entry,and SPSS24.0 statistical software was used for data analysis.Results After treatment,the total UPDRS score in the treatment group was significantly reduced(P < 0.001),and the daily activities and motor functions were decreased compared with the control group(P < 0.05).Among them,qi stagnation and blood stasis type and liver and kidney deficiency type were effective in treatment,phlegm heat and moving wind type showed an improvement trend,while qi and blood deficiency type showed no obvious improvement.The score of TCM senile fibrillation syndrome dysfunction was significantly reduced(P=0.001),and the curative effect was significant(P<0.001)since the 12 th week of medication,and the curative effect was proportional to the length of treatment(P=0.018).Qi-blood deficiency and hepatorenal deficiency were effective after 12 weeks of TCM treatment.The Hamilton anxiety scale(HAMA)score was significant(P=0.001),and the level of HAMA anxiety was significantly reduced after 12 weeks of medication(P=0.001).Among them,the total score of HAMA for qi-blood deficiency type was significantly decreased(P=0.003),the anxiety level of HAMA was significantly lower than that of the control group(P=0.007),the total score of qi-blood stagnation type was significantly decreased(P=0.024),and the anxiety level of HAMA was lower than that of the control group(P=0.030).The HAMD score of qi stagnation and blood stasis type was lower than that of other syndromes.The total score of RBD scale decreased(P=0.0272).Conclusion Pingzhan decoction is safe and effective in the treatment of motor symptoms and non-motor symptoms of Parkinson’s disease.It can effectively improve patients’ HAMA,HAMD,RBD,elderly fibrillation syndrome,UPDRS and other scales,and improve patients’ clinical symptoms and quality of life.Syndrome differentiation type pingzhan decoction is safe and effective in the treatment of motor and non-motor symptoms of Parkinson’s disease.
