The Immediate Effect Of Acupuncture On The "five Points Of The Lumbar Process" With Deep Needles On The Sensory Current Threshold Of Patients With Lumbar Intervertebral Disc Herniation

Objective:To investigate the characteristics of sensory current threshold in patients with lumbar disc herniation,and to evaluate the effect and safety of the treatment of "Five Points for LDH" with filiform needle.Methods:In this study,a prospective case series of 47 patients with unilateral lumbar disc herniation were included.All the patients were treated with the "Five Points for LDH" therapy with filiform needle.The main points of "Five Points for LDH" are Dachang Shu,Guan Yuanshu.L4 Huatuo Jiaji points.L5 Huatuo Jiaji points and S1 Huatuo Jiaji points.Point matching according to the patient’s condition:cold-dampness type plus Yaoyangguang;Wet heat type plus Sanjiaoshu;Blood stasis type plus Geshu;Deficiency of liver and kidney plus Shenshu,Before and after the intervention,the Current Perception Threshold and Pain Matching Threshold were measured by sensory nerve quantitative detector,and the pain intensity was measured by Visual Analogue Scale.Results:A total of 47 patients were included in this study.The Current Perception Threshold and Pain Matching Threshold were measured by 27 patients,and the pain intensity was measured by 47 patients.There was no statistical significance in the current threshold between the healthy side and the affected side in patients(P>0.05).In patients with unilateral herniated disc,the A-β fibers of the L5 nerve roots were both damaged.The mean current thresholds of A-δ fiber and C fiber in patients were within the normal threshold before and after intervention,and there is no statistically significant(P>0.05).There was no statistically significant in mean current threshold between male and female patients(P>0.05).There was no statistically significant in current threshold between young and middle-aged patients(P>0.05).There was no statistically significant difference in mean threshold between patients with different course of disease(P>0.05).After intervention,in patients with left disc herniation.the mean threshold of the L5 nerve root A-β fibers returned to normal.In patients with left disc herniation in health side,the average threshold before and after the intervention of the A-δ fiber and C fiber of L4 nerve root.C fiber of L5 nerve root,and A-β fiber and A-δ fiber of S1 nerve root was statistically significant(P<0.05),and in the projecting side,the average threshold before and after the intervention of A-β fiber of S1 nerve root was statistically significant(P<0.05).After intervention.the pain matching threshold of three frequencies were lower than that before intervention,and the threshold has no statistically significant(P>0.05),and the score of VAS was lower than that before intervention,and the difference was statistically significant(P<0.05).Conclusion:A herniated disc may affect both side of the sensory fibers.There was no statistically significant in current threshold between male and female patients(P>0.05).There was no statistically significant in current threshold between young and middle-aged patients(P>0.05).There was no statistically significant in sensory current threshold(P>0.05).The "Five Points for LDH" treatment has immediate analgesic effect.which can intervene to A-β fiber,A-δ fiber,and C fiber.
