| Backgroud:Psoriasis is a chronic immune-mediated inflammatory skin disease.The latest researches show that patients with psoriasis have subclinical systemic inflammation,which are associated with metabolic diseases such as diabetes and cardiovascular diseases.Studies had also proven that the joint involvement and the elevated incidence of CVD is also associated with the systemic inflammation in patients with psoriasis.Psoriasis is now considered as an independent risk factor for the occurrence and development of CVD.Many studies have reported that peripheral blood cells are closely associated with systemic inflammatory diseases such as CVD,but the correlation between peripheral blood cells with psoriasis has not been investigated.Here we study the peripheral blood cell amounts in patients with psoriasis and the control in order to find a simple and effective inflammatory markers of psoriasis,and to provide references for evaluating the severity of psoriasis and the effectiveness of clinical treatments.At the same time,it is of great significance in understanding the pathogenesis of psoriasis.Objective;To explore the correlation between pathogenesis of psoriasis and abnormality of peripheral blood cell amounts.Methods:Japan’s Sysmex XN-20 automatic flow cytometer and its original reagents and quality control products were used to do the blood test in psoriasis group and healthy control group.Results:(1)Peripheral blood WBC and psoriasis.①All parameters of white blood cells in psoriasis vulgaris with blood-heat syndrome group were significantly higher than those in healthy control group(WBC:7.25±2.18 vs 6.20± 1.60,P:0.000;NEU:4.35± 1.82 vs 3.71±1.20,P:0.000;LYM:2.08±0.65 vs 1.86±0.63,P:0.000;MON:0.56±0.18 vs 0.45±0.15,P:0.000;EOS:0.21±0.19 vs 0.14±0.13,P:0.000;BAS:0.041±0.028 vs 0.034±0.022,P:0.009).The difference between the two groups were statistically significant(P<0.05).②The MON,BAS amounts in peripheral blood of severe psoriasis group were significantly higher than those in mild to moderate psoriasis group(MON:0.59±0.18vs0.51±0.16,P:0.000;BAS:0.044±0.03±vs0.036±0.021,P:0.031),The differences between the two groups were statistically significant(P<0.05).(2)Peripheral blood RBC and psoriasis① The MCV,MCH and RDW-CV in peripheral blood of severe psoriasis group were significantly higher than those in mild to moderate psoriasis group(MCV:91.27±4.53 vs89.02±4.39,P:0.000;MCH:30.79± 1.69vs30.09± 2.04,P:0.004;RDW-CV:12.75±0.88vs12.5 7±0.99,P:0.036),the RBC amounts in peripheral blood of severe psoriasis group was significantly lower than that in mild to moderate psoriasis group(RBC:4.59±0.51vs4.75±0.51,P:0.015),The differences between the two groups were statistically significant P<0.05).(3)Peripheral blood PLT and psoriasis①The PLT,MPV and PCT in peripheral blood of psoriasis vulgaris with blood-heat syndrome group were significantly higher than those in healthy control group(PLT:246.15±64.52vs 233.73±6 1.66,P:0.040;MPV:10.33±0.99vs9.98±1.47,P:0.003;PCT:0.25±0.06vs0.23 ± 0.06,P:0.002),the PDW in peripheral blood of psoriasis vulgaris group with blood-heat syndrome was significantly lower than that of healthy controls(PDW:11.95±2.13vsl3.57±2.98,P:0.000),The differences between the two groups were statistically significant(P<0.05).(4)Peripheral blood NLR,PLR and psoriasis①The PLR in peripheral blood of psoriasis vulgaris with blood-heat syndrome group was significantly lower than that of healthy controls(PLR:129.39±51.96vs137.39±51.64,P:0.041),The differences between the two groups were statistically significant(P<0.05),while there was no definitely differences of NLR between the two groups(NLR:2.35±1.43vs2.20±1.01,P:0.733).②The NLR and PLR in peripheral blood of severe psoriasis group were significantly higher than those in mild and moderate psoriasis group(NLR:2.57±1.66vs2.00±0.88,P:0.006;PLR:137.74±56.38vs116.51±41.38,P:0.001),The differences between the two groups were statistically significant(P<0.05).(5)There was a significant positive correlation between PASI score and NEU,NLR,MON,EOS,MCV,MCH,PLR in psoriasis vulgaris with blood-heat syndrome group(The correlation coefficients were 0.153,0.218,0.280,0.194,0.272,0.208,0.225,P values were 0.018,0.001,0.000,0.003,0.000,0.001,0.000),while PASI score were negatively correlated with LYM,RBC(the correlation coefficients were-0.146,-0.219,P values were 0.024,0.001)(6)There was a significant positive correlation between PASI score and EOS,MON in the mild to moderate psoriasis group(the correlation coefficients were 0.253,0.323,P values were 0.014,0.002).(7)There was a significant positive correlation between the PASI score and NLR,PLR,RBC in severe psoriasis group(the correlation coefficients were 0.233,0.174,0.183,and P values were 0.005,0.036,0.028),while PASI score were negatively correlated with LYM,HGB,HCT,RBC(the correlation coefficients were-0.179,-0.212,-0.180,-0.236,P values were 0.031,0.011,0.030,0.000,0.004)(8)There was a significant linear regression connection between PASI and NLR in psoriasis vulgaris with blood-heat syndrome group(the regression coefficient was 2.784,P value was 0.001)(9)There was a significant linear regression connection between PASI and MON,EOS in the mild to moderate psoriasis group(the regression coefficients were 3.997,2.603;P values were 0.015,0.03 7).(10)There was a significant linear regression connection between PASI score and NLR in severe psoriasis group(the regression coefficient was 2.157,and P value was 0.009)(11)MCV,RDW-CV and PLR(MCV:OR=1.510,P=0.022;RDW-CV:OR=10.971,P=0.022;PLR:OR=1.012,P=0.035)were risk factors for the development and severity of psoriasis in psoriasis vulgaris with blood-heat syndrome group,while PDW(PDW:OR=0.706,P=0.047)was a protective factor.Conclusion:The MON and EOS amounts of peripheral blood may be associated with the severity of mild to moderate psoriasis.While the NLR of peripheral blood may be associated with the severity of severe psoriasis.The increase of MCV,RDW,PLR in peripheral blood may indicate that patients with psoriasis will get worse.However,the increase of PDW may suggest that patients with psoriasis will improve. |
