Background:Migraine is a common clinical disease characterized by one-sided head pain.Acupuncture is one of the commonly used traditional Chinese medicine treatment methods.It has been widely used in the clinical treatment of migraine.Firstly,is acupuncture actually effective for migraine?②Secondly,if it is effective,What are the high-frequency acupuncture point based on TCM syndrome differentiation?③Does the application of strengthening and reducing by needling have an effect on the clinical efficacy?Therefore,it is extremely necessary to study these three problems.In this way,the key problems in the clinical efficacy of acupuncture in treating migraine can be solved systematically,and valuable clues and basis can be provided for the further development of scientific research.Objective:(1)To Compare the efficacy and side effects of acupuncture and western medicine in treating migraine,and evaluate the effectiveness and safety of acupuncture in treating migraine;(2)To count the frequency of acupuncture point with different syndrome types of migraine,and summarize the use law of high-frequency acupoints;(3)To conduct data mining study on the efficacy of different acupuncture techniques of strengthening and reducing for the treatment of migraine with different syndromes,and evaluate the impact of strengthening and reducing by needling for migraine.Method:Chinese database:with“acupuncture”and“migraine”as the key words and the databases include CNKI,VIP,and Wan Fang database.English databases:with" acupuncture" "migraine" as the key words,the databases include PubMed,Medline,OVID,and Cochrane Library database.The searching time ranges from January 2000 to December 2018(19 years).(1)The literature of acupuncture therapy for migraine for nearly 19 years will be collected and sorted out and selected according to inclusion and exclusion criteria.Meta analysis will be performed using Revman5.3 software.(2)To retrieve the acupuncture treatment of migraine during the period of nearly 19 years related literature,the literature of acupuncture points in the prescription in the three kinds of syndrome types of migraine classified(Ascending hyperactivity of liver Yang,Phlegm wet blocking the meridians,Blood stasis),The frequency of acupuncture points of the three types was counted,and the correlation between acupuncture points and meridians,the use of specific acupuncture points and the parts of acupuncture points will be counted and analyzed.(3)the literature on strengthening and reducing by needling for treating migraine during the 19 years will be summarized and sorted out,and the data will be statistically processed by SPSS software to analyze whether strengthening and reducing by needling(lifting and thrusting,twisting)would have an effect on the efficacy of migraine.Results:This study was divided into three parts:1.In the Meta analysis of the clinical efficacy of acupuncture and western medicine in the treatment of migraine,the results showed that:(1)the clinical efficacy of acupuncture group vs.western medicine group was statistically significant(P<0.05),and acupuncture was effective in the treatment of migraine.In the meta-analysis of adverse reactions in acupuncture group vs.western medicine group,there was no statistically significant difference in gastrointestinal symptoms such as nausea/vomiting between the two groups(P>0.05).In terms of fatigue/weakness,drowsiness and overall adverse reactions,the difference between acupuncture group and western medicine group was statistically significant(P<0.05),and the incidence of adverse reactions of acupuncture group was lower than that western medicine group.2.In data mining studies on the frequency of acupuncture points used to treat migraine,by the data mining studies on the frequency of acupuncture on migraine acupuncture points,(1)Acupuncture points selection by disease differentiation:the most frequently used arupoints(top 5)were GB20(87),followed by GB8(77),EX-HN4(68),GV20(40)and LI4(32).Acupuncture points selected for syndrome differentiation:type of hyperactivity of liver Yang:the most frequently used acupoints(top 5)were LR3(90);LR2(41),GB34(41),KI3(34),and GB40(29).Phlegm wet blocking the meridians acupuncture points with high frequency were ST40(106),SP9(38),CV12(34),ST36(21)and PC6(7).Blood stasis type:the most frequently used acupoints were SP10(57),BL17(49),SP6(37),LI4(30),and LR3(18).Overall acupoints(main points+matching points):the most frequently used acupoints(the top 5 points)were LR3(132),GB20(126),LI4(102),ST40(108)and EX-HN4(86).(2)Acupoints-meridian in correlation analysis:acupuncture treatment of migraine of prescription of acupoints,all involved 14 channels and collaterals,acupoints of meridian(top 5)commonly used for The Gall Bladder Channel of Foot-Shaoyang(450),The Liver Channel of Foot-Jueyin[(182),The Stomach Channel of Foot-Yangming(180),Shaoyang Sanjiao Channel of Hand(164),The Spleen Channel of Foot-Taiyin(163).(3)association analysis of specific points:the most frequently used specific points are the crossing points(585),followed by the Five Shu points(514),the yuan-primary points(279),the fifteen collateral acupoints(206)and the eight influential points(142).(4)Correlation analysis of acupoints points:the head and neck were the most(32),followed by the lower extremity(24),upper extremity(12),back(2)and chest and abdomen(2).3.Data mining technology was used to analyze the differences in the therapeutic effects of strengthening and reducing by needling for treating migraine.It shows that no matter from the overall perspective or from the perspective of syndrome differentiation,the differences in the therapeutic effects between explicit strengthening and reducing by needling and non-explicit strengthening and reducing by needling were not statistically significant.At the same time,among the 38 literatures that met the inclusion criteria,13.16%(5/38)did not give a specific description and report on strengthening and reducing by needling.Conclusion:1.Evaluation from the perspective of acupuncture efficacy:acupuncture is effective and safe in treating migraine.2.Analysis from the perspective of acupoint use:(1)When acupuncture is used to treat migraine,acupuncture points selection by disease differentiation:the most frequently used acupuncture points were GB20,GB8,EX-HN4,GV20 and LI4.Acupuncture points selected for syndrome differentiation:Ascending hyperactivity of liver Yang:the most frequently used acupuncture points were LR3,LR2,GB34,KI3,and GB40.Phlegm wet blocking the meridians acupuncture points with high frequency were ST40,SP9,CV12,ST36 and PC6.Blood stasis type:the most frequently used acupuncture points were SP.10,BL17,SP6,LI4,and LR3.Overall acupuncture points(main points+matching points):the most frequently used acupuncture points were LR3,GB20,LI4,ST40 and EX-HN4.(2)The effect of Yang meridian on treating migraine with acupuncture is emphasized,especially shaoyang meridian.(3)Yang meridian should be emphasized when acupuncture is used to treat migraine.(4)Local and distal acupuncture points should be emphasized to improve clinical efficacy.3.Evaluation from the perspective of strengthening and reducing by acupuncture:(1)the therapeutic effect of strengthening and reducing by needling(lifting and thrusting,twisting)on different types of migraine may be limited.(2)the clinical research literature reports on strengthening and reducing by needling need to be further standardized. |
