The Clinical Study Of Jiawei Zhenwu Decoction On Chronic Heart Failure With Diuretic Resistance In The Syndrome Of Yang Deficiency And Water Flooding

Objective: Observe the clinical efficacy of Jiawei Zhenwu Decoction in the treatment of chronic heart failure with diuretic resistance in patients with yang deficiency and water syndrome,and provide more evidence-based medical evidence for TCM for chronic heart failure with diuretic resistance.Methods:Sixty patients who met the inclusion criteria were randomly divided into a treatment group and a control group,30 in each group.The control group was treated with Western medicine alone,and the treatment group was treated with Jiawei Zhenwu Decoction based on conventional Western medicine treatment.After 2 weeks of treatment,the 24-hour urine volume,body weight,NT-pro BNP,left ventricular ejection fraction(LVEF),NYHA cardiac function grade,TCM syndrome efficacy and other indicators of the treatment group and the control group before and after medication were observed.Through statistical analysis,observe the efficacy of the treatment group and the control group.Results: 1.Before treatment,there was no statistically significant difference in 24-hour urine volume,body weight,NT-pro BNP,left ventricular ejection fraction(LVEF),NYHA cardiac function classification,and total score of TCM syndromes(p>0.05),Comparable.Compared with before treatment,the 24-hour urine volume,body weight,NT-pro BNP,left ventricular ejection fraction(LVEF),NYHA cardiac function score,and total score of TCM syndromes of the two groups of patients improved after treatment,and the difference was statistically significant Significance(p<0.05).2.In terms of 24-hour urine volume and body weight: the urine volume of the treatment group and the control group increased significantly and the body weight decreased significantly after treatment at 24 hours.3.Furosemide reduction rate: the treatment group and the control group after treatment had a furosemide reduction rate of 80.00% and 60.00%,respectively,the treatment group was higher than the control group,the difference was statistically significant(p<0.05).4.In terms of NT-pro BNP and left ventricular ejection fraction(LVEF):NT-pro BNP and left ventricular ejection fraction improved in both groups after treatment(p<0.05),but no significant difference between the two groups after treatment(p>0.05).5.NYHA cardiac function grading: The total effective rate of NYHA cardiac function grading after treatment in the treatment group and the control group was 86.67% and 63.33%,respectively.The therapeutic effect of the treatment group was higher than that of the control group,the difference was statistically significant(p<0.05).6.The curative effect of TCM syndromes: The total effective rates of TCM syndromes in the treatment group and the control group after treatment were 86.67% and 63.33%,respectively,and the difference was statistically significant(p<0.05),suggesting that the treatment group was superior in improving TCM syndromes In the control group.Conclusion:Jiawei Zhenwu Decoction can improve chronic heart failure accompanied by diuretics in patients with yang deficiency water syndrome,24-hour urine output,weight,NYHA cardiac function classification,TCM syndromes,and the efficacy is better than Western medicine alone.It can also reduce the dosage of diuretics and reduce adverse drug reactions.There was no significant difference in NT-pro BNP and left ventricular ejection fraction(LVEF)between the two groups.
