| Purpose:This study objectively evaluated Professor Wang Xuefeng’s improvement of clinical symptoms in patients with narcolepsy due to deficiency of earth and excess of wood treated with prescriptions Zhouwu Decoction and Wuming Decoction(self-designed)in the morning and evening based on the theory of "Wake up during the day and sleep at night",and evaluated the clinical efficacy of the prescriptions in the treatment of narcolepsy.To explore whether it can improve the clinical efficacy,shorten the course of treatment,and form a new comprehensive treatment scheme of traditional Chinese medicine for narcolepsy,so as to provide new ideas and medication methods for clinical treatment of narcolepsy.Material and method:Collecting 60 children who were diagnosed as narcolepsy from December 2016 to December 2018 in pediatric clinic of Affiliated Hospital of Liaoning University of Traditional Chinese Medicine,and selecting 60 children who met the inclusion and exclusion criteria of this study,the information was collected by using "Symptom Evaluation Integral Scale of Chinese and Western Medicine for Narcotic Sleep Disease(self-developed)","Pittsburgh Sleep Quality Index(PSQI)" and "ESS Sleepiness Scale".The information collected included the general basic information of the children,the clinical symptoms in the table,and the clinical symptoms of each visit were recorded in the form of integrals in the scale.Sixty children were randomly divided into the treatment group(30 cases in the treatment group of "Zhouwu Decoction and Wuming Decoction")and the control group(30 cases in the treatment group of "Xingmei Decoction").Both groups took the medicine in the morning and evening.The treatment group took the first prescription in the morning,two days in the morning,and the second prescription in the evening,two days in the evening.The control group took the first prescription in the day.The two groups were treated for three courses.After the treatment,the changes of PSQI score,narcolepsy disease score,TCM syndrome score and ESS scale score were observed.The collected data were recorded and analyzed with EXCEL form and statistical software SPSS 20.0.To sum up the clinical experience of the tutor in the treatment of narcolepsy,and to elaborate the tutor’s understanding of the disease in TCM and the therapeutic methods and principles of using Zhouwu Decoction and Wuming Decoction in the treatment of narcolepsy due to hyperactivity of earth and wood in the morning and evening based on the theory of "nighttime and essence" and to evaluate the clinical efficacy.Results:(1)Baseline data: Sixty children with narcolepsy due to hyperactivity of soil deficiency and wood were eventually included in this study.They were randomly divided into two groups:treatment group(30 cases)with Dijue Fang and Night Gong Fang,and control group(30cases)with Xingmei Tangkou.The average age of the treatment group was 10.90+0.700 years old,18 males and 12 females,with an average course of 31.67+3.052 months;the average age of the control group was 10.47+0.615 years old,20 males and 10 females,with an average course of 37.00+3.827 months.After examination,there were no significant differences in gender,age,course of disease,PSQI scale score,ESS scale score,disease score of narcolepsy and TCM symptom score between the two groups.Comparability of the significance of accounting(P > 0.05)(2)TCM symptom score: After treatment,the scores and total scores of the treatment group and the control group were lower than those before treatment,and the difference was statistically significant(P < 0.05).After treatment,the scores of daytime sleepiness,night wakefulness and hand-foot tremor in the treatment group were 0.87(+0.229),1.00(+0.209)and 0.47(+0.157)respectively,while those in the control group were 1.67(+0.216),1.67(+0.194)and 1.00(+0.186)respectively.There were differences between the two groups(P <0.05),indicating that the treatment group was superior to the control group in terms of daytime sleepiness,night wakefulness and hand-foot tremor.The control group.(3)The main disease scores of narcolepsy: After treatment,the major disease scores and total scores of treatment group and control group were lower than those before treatment,and the difference was statistically significant(P < 0.05).After treatment,the scores of excessive daytime sleepiness,waking times and time increase in the treatment group were 0.87(+0.229),1.00(+0.209)and 0.47(+0.157)respectively,while those in the control group were 1.67(+0.216),1.67(+0.194)and 1.00(+0.186)respectively.There were differences between the two groups(P < 0.05),indicating that the treatment group was improving the clinical quadruples of narcolepsy children.Excessive daytime sleepiness,increased number and time of awakening and abnormal sleep were better than those in the control group.(4)PSQI score: After treatment,the PSQI scores and total scores of the treatment group and the control group decreased compared with before treatment,the difference was statistically significant(P < 0.05).After treatment,the scores of sleep quality,time to sleep,time to sleep,time to sleep,time to sleep,efficiency,and day dysfunction in the treatment group were 0.67(+0.121),0.13(+0.063),0.37(+0.089),0.80(+0.074),0.97(+0.089)and 3.27(+0.172)respectively,while those in the control group were 1.13(+0.093),0.40(+0.091),0.87(+0.093),1.03(+0.076)respectively.The dysfunction score was 1.43(+0.104)and the total score was 5.33(+0.273).There was a difference between the two groups(P < 0.05).It showed that the treatment group was superior to the control group in sleep quality,sleeping time,sleep efficiency,daytime dysfunction and PSQI reduction,which could improve the sleep quality at night.Conclusion:(1)Both Zhouwu Decoction and Wuming Decoction and Xingmei Decoction are effective prescriptions for the treatment of narcolepsy in children.(2)Generally speaking,the oral treatment of Zhouwu Decoction and Wuming Decoction is better than that of Xingmei Decoction.(3)Based on the theory of "Wake up during the day and sleep at night",the tutor has a practical clinical effect in treating narcolepsy of earthy deficiency and hyperactivity of wood with prescriptions Zhouwu Decoction and Wuming Decoction sooner or later,which provides a new method and treatment ideas for narcolepsy,and is worthy of further popularization and application. |
