| Objective:The optic nerve atrophy(OA)is a kind of disease because ofthe dysfunction of visual conduction from the optic nerve fiber,retinal ganglion cell and axon.The main clinical manifestation is that the visual acuity is reduced or even the blindness,the visual field defect,and the pale color in the papilla from the eye fundus examination.In the past,the necrosis of optic nerve was considered to be irreversible and the optic atrophy was the final outcome of a variety of diseases,once it had occurred,could not be reversed,and it was said that the disease had no therapeutic significance and value.But the most recent study says that,the atrophy of optic nerve is the clinical sign that is present in the course of the pathological damage of the nerve fiber.Even through the completely atrophied nerve fiber is irreversible,it is possible to stop the nerve fiber beingdeterioratedand to protect the non-damaged nerve fibers in the pathological process.If you actively searching for the cause,treating the original disease as early as possible,or using the combinationtreatment of the traditional Chinese medicine and western medicineinclinic,you can at least protect the visual function from further damage.So the active intervention treatment is an opportunity to improve the visual function and even to keep the visual function.The combination of traditional Chinese medicine and western medicine is a treatment method whic is highly recommended by clinicians.In order to obtain a high-level clinical evidence,this study adopts a prospective self-control research method to find the differentcurative effect of the acupuncturetherapyto treat the optic atrophy from different causes,which provided datum for further study of the mechanism of acupuncture.Method:1.60 patients with the diagnosis of optic atrophy were included in the ophthalmic clinic of the Oriental Hospital of Beijing University of Traditional Chinese Medicine.The patients were classified into each test group according to the cause of the disease.The patients in each group were treated with the Mr.Wei’sneedle therapy and the retain the needles for 30 minutes.Before and after treatment for 14 days and 28 days,theresultsof vision,visual field and visual evoked potential were recorded separately,the difference before and after treatment were analyzed.Furthermore,comparedpatients’subjective symptoms before and after the treatment.Results:There were 61 cases(89 eyes)were included.37 cases(58.7%)were male and 26 cases(41.3%)were female.The included patients were divided into 7 subgroups according to different causes.5 cases were open-angle glaucoma(10 eyes),13 cases were ischemic optic neuropathy(18 eyes),16 cases were traumatic optic neuropathy(16 eyes),and 9 cases were optic neuritis(17 eyes),6 cases(11 eyes)were genetic correlation,5 cases(9 eyes)weredescending optic nerve atrophy,7(8 eyes)were unknown reason.1.There were 3 eyes(3.37%)whichisexcellent,54eyes(59.55%)were effetiveand 32 eyes(37.08%)were ineffective.The total effective rate was 60.67%.After 4 weeks of treatment,the visual,visual and electrophysiology were significantly improved after 4 weeks of treatment(P=0.003<0.05).Acupuncture for ischemic optic neuropathy have the best efficiency,followed by unexplained optic atrophy and optic atrophy caused by open-angle glaucoma.In addition,the shorter the course,the better the curative effect,and the greater the visual acuity,suggesting that early detection and treatment will help improve visual function.Conclusion:Compared with before and after 4 weeks of treatment,the visual,visual field andvisual evoked potential of mostpatients with optic atrophy were significantly improved,which was suggested that acupuncture could improve and save the non-damaged optic nerve fibers.Theacupuncture priscription has the function of protecting the optic nerve,which is helpful to the improvement of the visual function.And the treatment is simple and easy to operate,which is feasible in clinical and has good clinical value. |
