Compilation Of The Competency Evaluation Scale For Emergency Physicians In County-level General Hospitals

Objective:On the basis of the competency model of emergency physicians constructed in the early stage,a set of scale of emergency physicians in the county-level comprehensive hospital was developed,and the reliability and validity of the scale were tested.This scale provides a scientific and effective tool for evaluating the competency of emergency physicians in county general hospitals,and lays a solid foundation for the next phase of nationwide at the county level comprehensive hospital emergency physician competence evaluation.Methods:(1)Through three rounds of research group discussions,combined with the actual situation of emergency physicians in county level general hospitals,we adjusted the index system of early stage and form a framework of scale.(2)Based on the Accreditation Council for Graduate Medical Education emergency assessment scale,the initial scale was drawn up according to the European Society for Emergency Medicine training course,the CanMEDS model and the general hospital emergency physician competency model.(3)The experts of management and emergency department were invited to conduct two rounds of expert consultation,and the competence index system was modified and perfected.Using questionnaire survey to 12 experts to evaluate the competency index name and definition?level 5 division and the description of the content,we discuss the discrimination basis and understanding of the experts on the content of the investigation.(4)Three hospitals were selected for pre-investigation.One index was deleted according to the statistical analysis results,and the final evaluation scale was obtained,with a total of 20 indicators.(5)The initial scale includes 6 dimensions and 21 indicators:emergency service,emergency operation skills,emergency and related knowledge,communication and communication skills,management coordination ability and internal quality.A total of 263 emergency doctors in 12 county general hospitals in Beijing were investigated.The reliability and validity of the test scale were tested.The reliability test was based on the homogeneity reliability and split-half reliability reliability,and the validity test was based on the content validity and construct validity.(6)The survey data of the effective questionnaire were introduced into the SPSS statistical software,and the statistical analysis was carried out by AMOS 21.0.Results:(1)By combing the domestic and foreign literature,we based on Accreditation Council for Graduate Medical Education milestone,the European Society for Emergency Medicine training course,the Royal College of Physicians and Surgeons of Canada competency model,tertiary hospital emergency physicians competency index system,the county level comprehensive hospital emergency physician competency model as the research foundation and compiled the initial scale.(2)We issue 13 county general hospital emergency department directors with confirmed questionnaire.The enthusiasm of the experts was very high and we take back all revised questionnaire.The effective recovery rate of the questionnaire and expert authority coefficient is 100%.We can make the conclusion that the authority of experts is better,and the evaluation reliability is higher.(3)We took back 263 valid questionnaires.After the reliability and validity testing,six dimension of homogeneity reliability Cronbach's a coefficient is 0.749-0.964,scale overall Cronbach's a coefficient is 0.962;Six dimension of split-half reliability is 0.724-0.946,scale overall split-half reliability is 0.919;It shows that the scale hao high reliability.(4)In the process of confirmatory factor analysis,the model's RMSEA is 0.075.The absolute fitness index,the value-added adaptation index and the simple fitness index all reach the standard.The hypothesis model can be accepted,and the fit degree of the model is good,indicating that the scale has good construct validity.Conclusion:(1)The dimensions,items and levels of the scale are reasonably,and the results of the study are reliable.(2)In the confirmatory factor analysis,six dimensions of the emergency physician competency assessment scale for the county general hospital are correspond to the actual data.The scale has good reliability and validity,which can be used to evaluate the competency of emergency physicians in county-level comprehensive hospitals.(3)The development of the scale makes the competency model build from the theory to the practical stage,and can be used as a tool for evaluating the competence of the scale in the next stage.The research has opractical significance.
