Purpose Mesiodens is a common clinical finding among children. The cause of Mesiodens is still inconclusive. It mostly occurs at the stage of mixed dentition period.Mesiodens can cause permanent incisors’ delayed or ectopic eruption, which can further change the occlusion and appearance of children. Therefore, the early diagnose of mesiodens in development is important for the clinician to allow for the most suitable treatment. The timing of mesiodens’ removal remains controversial.Mesiodens can cause permanent incisors’ delayed or ectopic eruption, which can further change the occlusion and appearance of children. Therefore, the early diagnose of mesiodens in development is important for the clinician to allow for the most suitable treatment. In this research,we observed the CBCT images of mesiodens in children from 3D perspective and analyzed the relationship between mesiodens’ growth direction, positions in different planes and different complications of mesiodens. Methods A total of 106 cone-beam computed tomography(CBCT)scans, taken between June 2014 and January 2020,from the Dental Center of The Affiliated Hospital of AHMU were screened.Mesiodens were classified according to their growth direction, positions in frontal, sagittal and axial planes.Complications caused by mesiodens were also rec-orded.The collected data were fed into SPSS 22.0 followed by conducting Spearman product moment correlation to detect the relationship between complications and positions. Results A total of 150 mesiodens were diagnosed in 106 children with ages ranging from 4– 11.Different complications were found in 72 mesiodens. The majority of the mesiodens were mostly found near midlines or in between of central incisors, unilateral located in the premaxillary region, conical shaped, and unerupted. The results of Spearman correlation revealed associations between complications and positions in coronal plane, sagittal plane and axial plane(P values were <0.001,0.030,<0.001).Conclusions: There are correlations between positions of mesiodens and complications. The use of CBCT is essential for both diagnosis and treatment planning in mesiodens. If it is judged that the position of the mesiodens will affect the eruption of the anterior teeth,it is recommended to perform surgical treatment of the supernumerary tooth extraction as soon as possible and supplemented with further repair after the operation;if the position of the mesiodens interferes with the eruption of adjacent teeth slightly,or surgery may cause greater trauma to the child, the early operate is not optional, continue observing and conservative treatment are more advised.Cone-beam computed tomography provides more detailed information about position, neighbouring anatomic structures, and local findings of the presence of mesiodentes in multiply sections.Early diagnosis of mesiodens allows the most suitable treatment. With the help of general examination and evaluation by CT imaging, the possibility of damage to adjacent permanent teeth during early surgical intervention can be minimized. But concerned of the special radiation health effect among children, the use of CBCT in children should be prudent.
