Objective:To investigate the clinical efficacy of the treatment of the overlap syndrome in GERD and IBS-D(syndrome of cold-heat complex)by Chaihu Guizhi Ganjiang Decoction combined with rabeprazole.Methods: Sixty cases with IBS-D overlapping GERD disease that met the inclusion criteria were randomly divided into two groups: 30 cases in the treatment group and the others in the control group.The patients in the treatment group were treated with Chaihu Guizhi Ganjiang Decoction combined with rabeprazole enteric-coated capsules.Moreover the others in the control group were treated with rabeprazole enteric-coated capsules combined with trimebutine maleate.All groups were treated for 8 weeks.Observe and record the results of patients of the two groups,including IBS-SSS scale scores,GERD questionnaire scores and TCM syndrome scores.Results: 1.IBS-SSS results: before the treatment,the scores of IBS-SSS had no significant difference between two groups(P>0.05).After the treatment,scores of all cases in 2 groups were lower than those before the treatment(P<0.01).In comparison,the scores of the treatment group were lower than those of the control group significantly(P<0.01).The improvement of the IBS-SSS scale showed that the efficient rate in the treatment group was93.3%,and the efficient rate in the control group was 83.3%.The difference was statistically significant(P>0.05).They show that the treatment group is significantly better than the control group in improving the symptoms of IBS-D.2.Gerd Q questionnaire score: The scores of all cases between the two groups had no significant difference before the treatment(P> 0.05).After the treatment,the two groups’ scores were lower than those before the treatment(P <0.05).Between two groups,the results of scores had no significant difference after the treatment(P>0.05).The results show that both groups can improve the symptoms of gastroesophageal reflux disease effectively.Furthermore there is no significant difference between two treatment programs.3.Analysis of TCM symptom score results: with all of the patients in the treatment group,3 cases were markedly effective,26 cases were effective,1 case was ineffective,and the total effective rate was 96.7%.In the control group,all of the cases were treated,none of themwas markedly effective,26 cases were effective,4 cases were ineffective,and the total effective rate was 86.7%.The curative effect of the treatment group was superior to that of the control group.The difference was statistically significant(P<0.05).Before the treatment,the results had no significant difference between the treatment group and the control group(P>0.05).After the treatment,the TCM symptom scores of the two groups were significantly lower than those before the treatment(P<0.01).Compared the results of two groups,the TCM symptom scores of the treatment group were lower than those of the control group(P<0.05).These results show that the curative effect of the treatment group is significantly better than that of the control group in improving the overall symptoms of Traditional Chinese Medicine.The results of TCM main symptom scores: after the treatment,the idividual scores of TCM main symptoms in the two groups were lower than those before the treatment(P<0.05);between two groups,the TCM main symptom scores of the treatment group were lower than those of the control group(P<0.05).The results show that the curative effect of the treatment group was superior to that of the control group in improving stool thinness(P<0.05).But there was no significant difference between two groups in improving acid reflux or heartburn,and abdominal pain(P>0.05).The results of TCM secondary syndrome scores: the scores of the secondary syndrome after the treatment in two groups were lower than those before the treatment significantly(P<0.05).After the treatment,the scores of secondary symptoms in the treatment group were significantly lower than those before treatment(P<0.05).The scores of bitterness or bad breath,abdominal distension or bowel sounds in the control group were lower than before(P<0.05).There was no significant improvement in chills score(P>0.05).The treatment group was more effective in improving bitter and the afraid of cold than the control group(P<0.05).The results show that the curative effect of the treatment group is significantly better than that of the control group in improving TCM secondary symptoms.Conclusion: Both Chaihu Guizhi Ganjiang Decoction combined with rabeprazole and Trimebutine Maleate combined with rabeprazole have played a good role on treating IBS-D overlapping GERD.And the curative effect of Chaihu Guizhi Ganjiang Decoction combined with rabeprazole is more effective than that of Trimebutine maleate combined withrabeprazole.
