Objective:Chronic Subdural Hematoma(CSDH)is one of the common and frequently-occurring neurosurgical disease.According to statistics,its incidence is as high as(5-58)per 100,000,especially in the elderly.Surgery is still the first choice for the treatment of CSDH.Some patients still need to face more difficult problems after choosing surgical treatment,such as brain atrophy of varying degrees,postoperative expansion of brain tissue insufficiency,residual intracranial hematoma and gas accumulation risk,even the possibility of infection or death during perioperative surgical treatment.In addition,postoperative hematoma recurrence rate is still high,up to 5%-30%.In order to enhance the efficacy of surgery,reduce recurrence and improve quality of patients’ life,atorvastin,a commonly used lipid-lowering drug in clinical practice,has gradually come into public’s sight in recent years.Studies have shown that atorvastatin can effectively improve the prognosis of patients and reduce the risk of postoperative recurrence.At the same time,according to the theory of traditional Chinese medicine,the anti-therapeutic method of “Yi Tong Zhi Tong” in traditional Chinese medicine can be used for patients with CSDH of qi stagnation and blood stasis type.Modified Xuefu Zhuyu decoction is adopted to promote the absorption of hematomas and improve symptoms such as headache and dizyness in patients.This study will conduct a retrospective study of 60 elderly patients with CSDH of qi stagnation and blood stasis,and analyze the clinical efficacy of modified Xuefu Zhuyu decoction combined with atorvastin in the treatment of with CSDH,with a view to provide a more reasonable and effective treatment plan,as well as evidence-based medicine basis for the treatment of CSDH.Method:To ensure the reliability of the data and the validity of the experiments,this study will be divided into three parts for demonstration.Content 1: To conduct Meta-analysis of atorvastatin in the treatment of CSDH by searching the Chinese andEnglish literature database.Content 2: To conduct Meta-analysis of Xuefu Zhuyu decoction in the treatment of CSDH.Content 3: To collect clinical data of CSDH patients admitted to the Second Affiliated Hospital of the University of Traditional Chinese Medicine from January 2014 to January 2019.50 patients with CSDH who were included in the experiment were divided into two groups.The control group was routinely treated after surgery.Atorvastatin was added on the basis of this.Patients in the experimental group were treated with modified Xuefu Zhuyu Decoction and atorvastatin on the basis of conventional postoperative treatment.The residual hematoma volume,length of hospital stay,daily living activity scores,recurrence rates,and symptoms of traditional Chinese medicine of the two groups were compared and analyzed.SPSS 21.0 software was used for statistical analysis,in which difference was considered statistically significant when P <0.05.Result:1.Atorvastatin is safe and effective in treating CSDH.2.Xuefu Zhuyu Decoction is effective and safe in treating CSDH.3.The residual hematoma volume,length of hospital stay,and postoperative recurrence rate in the experimental group were significantly lower than those in the control group.And the scores of daily living activities and improvement in TCM symptoms were significantly better than those in the control group.The differences were statistically significant.(P < 0.05).Conclusion:Xuefu Zhuyu Decoction combined with Atorvastatin has a positive effect in treating patients with CSDH of qi stagnation and blood stasis type,which can effectively promote the absorption of hematoma,shorten the length of hospital stay,reduce the recurrence rate of surgery,improve the neuromotor function and the quality of life of patients after surgery. |
