Objective:To investigate the risk factors of recurrence of ischemic stroke and the relationship between Platelet endothelial aggregation receptor-1,Cyclooxygenase-1,Glycoprotein?a polymorphism and recurrence of ischemic stroke.Methods:From October 2018 to July 2019,76 patients with acute ischemic stroke who were treated in Department of Neurology of Hebei University Affiliated Hospital were selected according to inclusion and exclusion criteria,including 41 in the first group and 35 in the relapse group.Record all patients' sex,age,past medical history,medication history,smoking history,drinking history,improve blood urine stool routine,liver function,kidney function,blood lipid,coagulation function,fasting blood glucose,glycosylated hemoglobin,blood homocysteine,head CT,head MRI,head neck CT angiography,carotid vertebral artery subclavian artery ultrasound,echocardiography and other auxiliary examinations.Venous blood of all patients was collected 2-3ml for DNA extraction,and gene sequencing and single nucleotide polymorphism analysis were performed using fluorescence quantitative PCR.Single factor analysis was conducted to compare the differences between the two groups of clinical data,auxiliary examination results,allele frequency and genotype by t test and chi square test,while multi-factor analysis was conducted by logistics regression.Results:1.The detection of three gene loci in 76 patients showed: PEAR1(rs12041331)GG genotype in 20 cases,GA genotype in 46 cases,AA genotype in 10 cases,allele frequency of G was 56.6%,allele frequency of A was 43.4%;COX-1(rs1330344)CC genotype in 12 cases,CT genotype in 39 cases,TT genotype in 25 cases,allele frequency of C was 41.4%,allele frequency of T was 58.6%.GP?a(rs5918)TT genotype in 74 cases,TC genotype in 2 cases,CC genotype in 0 cases,allele frequency of T was 98.7%,allele frequency of C was 1.3%.2.The single factor analysis showed that the two groups of gender,average age,combined diabetes,low density lipoprotein cholesterol,PEAR1 allele frequency of A,AA+GA homozygous genotype frequency,AA genotype frequency and allele COX-1 C frequency,CC+CT genotype frequency difference was statistically significant(P <0.05),two groups of history of hypertension,coronary heart disease,smoking,drinking,triglyceride,total cholesterol,high-density lipoprotein cholesterol,blood homocysteine and carotid plaques and cerebral artery stenosis,COX-1 genotype CC,GP?a gene polymorphisms with no statistical significance(P>0.05).3.Multivariate regression analysis showed that diabetes mellitus,AA and AA+GA of PEAR1 gene,and CC+CT of COX-1 gene were significantly correlated with the recurrence of ischemic stroke(P<0.05).Conclusion:Age,gender,and low-density lipoprotein cholesterol were associated with the recurrence of ischemic stroke.Diabetes mellitus,PEAR1(rs12041331)allele A,COX-1(rs1330344)allele C are independent risk factors for recurrence of ischemic stroke.GP?a(rs5918)gene polymorphism was not associated with recurrence of ischemic stroke.
